原发性痛经中医证候分布特点探讨

摘　　要：目的:探讨原发性痛经发生的相关因素和常见中医证候,寻求原发性痛经的中医证候分布规律、证候特点,以及与患者发病因素、痛经程度等因素的关系。方法:采用原发性痛经中医证候信息采集表,对门诊符合条件的606例原发性痛经患者进行横断面调查。结果:本次调查研究发现原发性痛经以实证居多,实证中以寒湿凝滞证居多,气滞血瘀证次之;虚证中以气血虚弱证居多,同中医证候原发性痛经的疼痛程度也有差别;虚证的VAS评分偏小,实证的VAS评分偏大,即实证患者的痛经程度大于虚证患者;痛经患者在身体状况、疼痛程度、疼痛持续时间、伴随症状以及妨碍工作学习的程度,反映了痛经对患者的生理、心理健康方面均造成了明显的影响,说明疼痛是影响患者生活和学习及情绪的主要因素之一。结论:原发性痛经患者的中医证候主要有气滞血瘀证、寒湿凝滞证、气血虚弱证、肝肾亏损证,并以实证居多。原发性痛经患者的虚证的VAS评分偏小,实证的VAS评分偏大。Objective：To investigate the related factors and common TCM syndromes of primary dysmenorrhea,looking for TCM syndrome distribution laws,and syndrome characteristics of primary dysmenorrhea,and the relationship between them and factors such as pathogenic factors,degree of dysmenorrhea,etc.Methods：TCM syndrome information collection form of primary dysmenorrhea was used and 606 cases of patients with primary dysmenorrhea accordance with the conditions in outpatient were carried out cross-sectional survey.Results：The survey study found that primary dysmenorrhea was in the majority with excess syndrome,and excess syndrome was in the majority with stagnation of cold-dampness syndrome,and qi stagnation and blood stasis syndrome taking second place;Deficiency syndrome was in the majority with qi and blood weakness syndrome,and the degree of pain in the same TCM syndrome of primary dysmenorrhea were different.VAS scoring of deficiency syndrome was low,and that of excess syndrome was high,that is,dysmenorrhea degree in the patients of excess syndrome was greater than the patients of deficiency syndrome;Physical condition,the degree of pain,pain duration,accompanied symptoms and interfering with work and study of dysmenorrhea patients reflected that dysmenorrhea caused a significant impact on patients＂ physical and mental health,which explained that pain was the main factor affecting the patients＂ life,study and emotion.Conclusion：The main TCM syndrome type of primary dysmenorrhea is qi stagnation and blood stasis syndrome,stagnation of cold-dampness syndrome,qi and blood weakness syndrome,and liver and kidney deficiency syndrome,and in the majority with excess syndrome.VAS scoring of deficiency syndrome was low,and that of excess syndrome was high.
