肾病综合征患儿甲状腺素水平与中医证候相关性研究

摘　　要：目的:通过临床病例调查,分析肾病综合征(nephrotic syndrome,NS)患儿血清游离甲状腺素水平与中医证候的相关性。方法:选取肾病综合征患儿191例。根据甲状腺功能水平及治疗方案不同分为A组(伴甲状腺功能低下服用左甲状腺素片)、B组(伴甲状腺功能低下未服用左甲状腺素片)、C组(甲状腺功能正常未服用左甲状腺素片)、D组(甲状腺功能正常服用左甲状腺素片)四组。治疗后观察中医各证候甲状腺素的水平。结果:NS中医各证候甲状腺素的水平为,脾虚湿困兼血瘀证T3、T4明显下降,脾肾阳虚兼血瘀证TSH升高较其他证候明显(P<0.05)。肺脾气虚兼血瘀证、脾肾阳虚兼血瘀证分别与气阴两虚兼血瘀证在T3水平上存在显著性差异(P<0.01),余各证候在甲状腺素水平方面无明显差异(P>0.05)。结论:肾病综合征患儿血清游离甲状腺素水平与中医证候有一定的相关性。Objective：To analyze the correlation of thyroid hormone levels and TCM syndrome type of children with nephrotic syndrome through clinical cases investigation.Methods：191 cases of children with nephrotic syndrome were selected and were divided into group A（children accompanied with low thyroid function taking Levothyroxine Tablet）,group B（children with low thyroid function not taking Levothyroxine Tablet）,group C（children with normal thyroid function not taking Levothyroxine Tablet）,and group D（children with normal thyroid function taking Levothyroxine Tablet）.Results：The levels of thyroid hormone in TCM syndrome type of nephrotic syndrome：T3 and T4 of spleen deficiency and dampness obstructing combined with blood stasis syndrome were decreased obviously,and increase of TSH levels of spleen and kidney yang deficiency combined with blood stasis syndrome were clearer than other types（P0.05）.The differences of lung and spleen qi deficiency and blood stasis,and spleen and kidney yang deficiency and blood stasis respectively compared with deficiency of qi and yin and blood stasis in T3 level were statistically significant（P0.01）,and there were no remarkable difference in thyroid hormone levels among the rest of syndrome types（P0.05）.Conclusion：Thyroid hormone levels of children with nephrotic syndrome have certain relevance with TCM syndrome types.
