显微镜下多血管炎合并肺损害8例临床分析

摘　　要：目的探讨显微镜下多血管炎(microscopic polyangiitis,MPA)的临床特征和诊疗方法。方法对我院2008年1月-2012年12月间收治的MPA8例患者临床资料进行回顾性分析。结果 8例患者均有肺部损害,伴不同程度的多器官损害,7例患者出现白细胞升高、贫血、镜下血尿、蛋白尿、红细胞管型、肌酐及尿素氮升高,8例患者出现核周型抗中性粒细胞胞浆抗体(primary antineutrophilcytoplasmicantibodies,P-ANCA)阳性,6例患者出现抗髓过氧化物酶(myeloperoxidase,MPO)ANCA阳性。CT表现为双肺弥漫性磨玻璃影为主,4例患者出现肺间质纤维化。6例患者治愈或好转,2例患者恶化或死亡。结论MPA临床表现无特异性,多有肺损害,易误诊,ANCA对MPA的诊断有重要价值。Objective To investigate the clinical manifestation of microscopic polyangiitis（MPA）. Methods Eight microscopic polyangiitis patients complicated with pulmonary involvement from January 2008 to December 2012 were studied retrospectively, admitted to the Jiangmen Central hospital. Results Pulmonary involvement were presented in all patients,and renal symptom in seven patients. Chest CT lesions showed ground-glass opacity and pulmonary fibrosis. 6 out of 8 patients were cured or improved. All patients showed positive MPO-ANCA and 6 of P-ANCA. Conclusion The clinical manifestation of MPA were not specific, and most of the MPA patients complicated with pulmonary involvement. Antineutrophil cytoplasmic antibedy（ANCA） will be of great value to MPA＇ s diagnosis.
