对某院中草药处方不适宜用药干预记录表的分析

摘　　要：目的促进临床中草药使用的安全。方法采用回顾性分析方法,对某院中药房2011年1月至2012年11月不适宜用药干预记录表进行统计、评价和分析。结果不合理用药疑问处方1725张,占总处方量的1.5%。其中医生在处方上重签名至规范的有1237张,医生修改处方至合理的有467张,医生未修改处方或签名的有21张,分别占不合理用药疑问处方比例为71.7%,27.1%和1.2%,有效干预率达98.8%。结论该院中草药使用基本合理,但仍存在一定问题,主要存在用量过大,配伍禁忌,疗程过长等问题,更需要药师与临床医生加强沟通和干预,做到安全有效合理用药。Objective To promote the use of herbal medicine in clinical safety. Methods A retro-spective analysis of 2011 January to 2012,the pharmacy in a hospital in November is not suitable for drug inter-vention record statistics,analysis and evaluation. Results The irrational drug use is the total 1725 prescrip-tions,prescription dose of 1.5% . The doctors prescribe heavy signature to specification 1237,doctor modify the prescription to a reasonable 467,the doctor did not modify the prescription or signature has 21,accounted for the irrational drug use doubt prescription proportion is 71.7% ,27.1% and 1.2% ,effective rate was 98.8% . Conclusion The Chinese herbal medicine use is reasonable but there are still some problems of ex-cessive,incompatibility,problems of long duration,pharmacists and clinicians need to strengthen communica-tion and intervention,to achieve safe effective and rational use of drugs.
