摘 要:目的探讨氮硝基左旋精氨酸对烟雾吸入性损伤大鼠肺组织的保护作用。方法雄性Sprague-Dawle大鼠60只采用随机数字表法分为4组,空白组6只,余下分为生理盐水组、低剂量组和高剂量组,每组设6h、24h、48h三个时相点,各时相点6只。空白组不予任何处理,其余3组大鼠制作烟雾吸入性损伤模型,生理盐水组、低剂量组和高剂量组于伤后分别腹腔注射等容量的生理盐水、浓度为10mg/kg的氮硝基左旋精氨酸和浓度为20mg/kg的氮硝基左旋精氨酸。注射30min后将每只大鼠股动脉取血5mL,离心后检测血清白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)含量,活杀后取部分左肺匀浆后检测丙二醛(MDA)、髓过氧化物酶(MPO)及超氧化物歧化酶(SOD)含量,取部分右肺经4%甲醛溶液固定后做病理苏木精-伊红染色切片光镜观察。结果低剂量组48h与同时相点生理盐水组相比,TNF-α[(179.51±8.67)ng/L vs (231.39±9.98)ng/L],IL-6[(289.01±31.84)pg/mL vs(398.67±32.93)pg/mL],MDA[(4.37±0.62)nmol/mg vs (4.41±0.35)nmol/mg],MPO[(41.39±1.49)U/g vs (50.18±1.72)U/g]降低,差异有高度统计学意义(P<0.01),SOD[(93.82±5.69)U/mg vs (88.46±5.48)U/mg]和IL-10[(110.33±4.21)ng/L vs (108.85±5.47)ng/L]升高,差异有高度统计学意义(P<0.01)。高剂量组比低剂量组差异更显著。病理切片显示致伤后6h低剂量组和高剂量组大鼠相较生理盐水组肺组织炎细胞浸润减少,肺泡萎陷程度减轻,其中高剂量组比低剂量组改善更明显。结论氮硝基左旋精氨酸可减轻大鼠吸入性损伤后肺组织中炎性反应及过氧化反应,从而减轻肺损伤的程度。Objective To explore the protective effects of Nω-nitro-L-arginine (L-NNA) in lungs of rats with smoke inhalation injury.Methods Sixty male Sprague-Dawley rats were randomly divided into 4 groups,6 in blank group.The remaining rats were divided into saline group,low dose group and high dose group,and each group set 6 h,24 h,48 h three time points,6 rats in each time point.The rats without any treatment were put in blank group,the remaining three groups of rats were making models of smokeinhalation injury.Saline group,low dose group and high dose group after injury were injected with equivalent saline,the concentration of 10 mg/kg L-NNA and the concentration of 20 mg/kg L-NNA.Each rat took blood 5 mL after 30 minutes of injection,centrifuged and tested interleukin 6 (IL-6 ) ,interleukin-10 (IL-10) ,tumor necrosis factor α (TNF-α) content in serum,and tested malondialdehyde (MDA) , myeloperoxidase (MPO) and superoxide dismutase (SOD) content in lung,examined the lung tissue under microscope.Results Low dose group 48 h TNF-α (179.51 ± 8.67) ng/L vs (231.39 ± 9.98) ng/L, IL-6 (289.01 ± 31.84) pg/mL vs (398.67 ± 32.93) pg/mL,MDA (4.37±0.62) nmol/mg vs (4.41 ± 0.35) nmol/mg,MPO (41.39 ± 1.49) U/g vs (50.18 ± 1.72) U/g decreased compared to the saline group (P0.01) ,SOD (93.82 ± 5.69) U/mg vs (88.46 ± 5.48) U/mg and IL-10 (110.33 ± 4.21) ng/L vs (108.85 ± 5.47) ng/L decreased compared to the saline group (P0.01) .The discrepancy between high dose group and saline group was more statistically significant than low dose group.Pathological section display that there were fewer inflammatory cells and slightly reduced alveolar collapse in lung tissue of rat in low dose group and high dose group compared with saline group at 6 h after injury,and more significant improvements in high dose group than in the low dose group.Conclusion L-NNA can reduce inflammation and chemical reaction in the lung tissue of rats after inhaled injury
