阻塞性睡眠呼吸暂停综合征合并高血压患者内脂素水平与白天嗜睡的相关性研究

摘　　要：目的探讨阻塞性睡眠呼吸暂停综合征(OSAS)及阻塞性睡眠呼吸暂停综合征合并高血压患者血清内脂素水平变化,分析它与白天嗜睡程度、体质量指数和AHI的相关性。方法随机选择OSAS合并高血压组20例、OSAS组30例和正常对照组15例,均进行整夜7 h的多导睡眠呼吸监测,记录呼吸暂停低通气指数(AHI)和最低SaO2、平均SaO2。测量研究对象的身高、体质量及计算体质量指数(BMI)。酶联免疫法测各组血清内脂素水平。分析内脂素水平与嗜睡程度、BMI和AHI之间的相关性。结果①OSAS合并高血压组嗜睡程度(10.80±4.06)显著高于OSAS组(8.63±2.31)和正常对照组(3.50±2.07),差异有显著性(P<0.05)。②OSAS合并高血压组血清内脂素水平(45.94±8.63)pg/mL显著高于OSAS组(23.64±13.66)pg/mL和正常对照组(6.86±2.60)pg/mL,差异有显著性(P<0.01)。OSAS组血清内脂素水平显著高于正常对照组,差异有显著性(P<0.01)。③血清内脂素水平与嗜睡程度、BMI和AHI均呈显著正相关(r=0.530,r=0.605,r=0.578,P<0.05),与最低SaO2呈显著负相关(r=-0.637,P<0.05)。结论 OSAS和OSAS合并高血压患者均有使内脂素水平升高的趋势,在OSAS合并高血压患者中升高更明显。血清内脂素水平与嗜睡程度相关。To find out the roles of serum visfatin changes in disease progression of OSAS patients combined hypertension.【Methods】Randomly selected the group of patients with OSAS（20 cases）,OSAS patients combined hypertension（30 cases） group and normal control group（15 cases）.7 hours were carried out overnight polysomnography,recording AHI,the lowest SaO2 and the average SaO2.Each patient＇s height and weight were measured in order to calculate their BMI.Serum visfatin levels of each group was detected by ELISA and analyze the correlation between serum visfatin and sleepiness,BMI,AHI and blood pressure【.Results】The sleepiness of OSAS patients combined hypertension（10.80±4.06） was significantly higher than that of patients with OSAS（8.63±2.31） and the normal control group（3.50±2.07,P 0.05）.The serum visfatin levels of OSAS patients combined hypertension（45.94±8.63） pg/mL was significantly higher than that of patients with OSAS（23.64 ±13.66） pg/mL and the normal control group [（6.86±2.60） pg/mL,P 0.01].The serum visfatin levels of OSAS patients was significantly higher than that of the normal control group（P 0.01）.The correlation between serum visfatin levels and sleepiness,BMI and AHI was positively（r =0.530,r =0.605,r = 0.578,P 0.05）.The serum visfatin levels and the lowest SaO2 was negatively correlated（r =-0.637,P 0.05）.【Conclusions】Both patients with OSAS and OSAS patients combined hypertension had serum visfatin levels on the rise,exceptionally in OSAS patients combined hypertension.Serum visfatin levels associated with sleepiness.
