摘 要:目的对吉林市三级甲等医院儿科呼吸道感染标本分离的58株产超广谱β内酰胺酶(ESBLs)大肠埃希菌进行耐药基因型研究。方法采用聚合酶链反应(PCR)扩增对产ESBLs大肠埃希菌的初筛确证实验得到的产ESBLs大肠埃希菌58株,进行耐药基因型研究。结果 58株产ESBLs大肠埃希菌中,β-内酰胺酶基因型分别为blaSHV2a(2/58),blaTEM-1(7/58),blaCTX-M-15(17/58)。结论吉林地区儿科呼吸系统感染住院儿童对β-内酰胺类抗菌药物的耐药机制主要是由于产生TEM-1型广谱β-内酰胺酶,SHV2a型和CTX-M-15型超广谱β-内酰胺酶。Objective To study the genotypes of 58 Escherichia coli stroins producing ESBLs. The strains were cultured from infection specimens of the respiratory tract in pediatric department of Jilin City hospitals. Methods The'drug resistant genotypes of 58 strains of Escherichia coli producing ESBLs were analyzed by PCR. Results The results of β- lactamases genotype showed that two strains carried blassv2a, seven carried blaTEM-1 and seventeen carried blacTX.M_IS. Conclusion The mechanism of resistance to beta-lactam antibiotics in pediatric hospital resporatory infections in Jilin is partly due to the production of TEM-1 broad spectrum B-lactamase,SHV2a and CTX-M-15 extended spectrum β-lactamase.
