摘  要:目的探讨不同剂量依那普利对大鼠肢体缺血-再灌注后心肌细胞的作用。方法健康雄性SD大鼠60只,采用随机数字表法,将其随机分为5组(n=12):正常对照组,模型组,低、中、高剂量药物组。模型组和低、中、高剂量药物组以橡皮带环绕结扎大鼠双后肢根部3 h并经激光多普勒血流仪监测证实血流被阻断,正常对照组大鼠双后肢松绕橡皮带3 h。低、中、高剂量药物组经颈内静脉分别注入依那普利0.01,0.02,0.04 mg.kg-1,其他两组大鼠给予等量0.9%氯化钠溶液,30 min后,松开橡皮带。再灌注3 h后取心肌组织,TUNEL法测定心肌细胞凋亡,免疫组化法测定心肌组织Bcl-2、Bax蛋白表达,采用黄嘌呤氧化酶法和硫代巴比妥法测定超氧化物歧化酶(SOD)活性和丙二醛(MDA)含量。结果与正常对照组比较,其他组心肌细胞凋亡指数(AI)及心肌组织Bax蛋白表达和MDA含量升高,而Bcl-2蛋白表达及SOD活性降低(P<0.05)。与模型组比较,低、中、高剂量药物组AI及Bax蛋白表达和MDA含量降低,而Bcl-2蛋白表达及SOD活性升高(P<0.05)。结论依那普利可减轻大鼠肢体缺血-再灌注心肌损伤,但与剂量无关,其机制与抗氧化和抑制细胞凋亡有关。Objective To investigate the effects of different doses of enalapril on myocardial cells by ischemia reperfusion in limbs of rats. Methods Sixty healthy male SD rats were randomly divided into 5 groups ( n = 12 ) : normal control group, model control group and enalapril at low, medium, and high dose groups. The posterior limbs of the rats in the model and enalapril groups were ligatured by rubber bands, and the laser Dopp],er flowmetry was employed to confirm blockage of the blood flow. The posterior limbs of the normal control group were loosely banded with rubber band for 3 hours. The three treatment groups were given enalapril at the doses of O. 01,0.02 and 0.04 mg · kg^-1 via jugular injection, and the other two groups were given with equal amount of saline. The bands were loosened 30 min later. After reperfusing for 3 h, cardiomyocyte apoptosis was evaluated by TUNEL detection, the expression of Bcl-2 and Bax protein was determined by immunohistochemical method, and activity of SOD and content of MDA were detected by xanthine oxidase method and thiobarbituric acid method . Results Compared with the normal control group,, myocardial apoptosis index ( AI), myocardial tissue Bax protein expression and the content of MDA increased in other groups, while Bcl-2 protein expression and SOD activity decreased ( P〈0.05 ). Compared with the model group, myocardial apoptosis index (A1) Bax protein expression and MDA content decreased while Bcl-2 protein expression and SOD activity increased (P〈0.05) in the low, medium and high dose groups. Conclusion Enalapril may play a protective role on myocardial ceils by limb ischemia reperfusion in a dose-independent manner, the mechanism of which may be related to antioxidation and apoptosis inhibition.
