老年全髋置换患者术后隐性出血与生存质量关系的对照研究

摘　　要：目的探讨老年全髋人工关节置换患者术后隐性失血与生存质量的关系。方法对87例患者进行分析。运用Gross方程计算老年THA患者围手术期隐性失血数量,并按隐性失血量分为两组,高HBL组(A组),隐性失血量高于所有患者平均值;低HBL组(B组),隐性失血量低于所有患者平均值。记录各组患者术前、及末次随访时SH-36评测表,并记录患者Harris评分。结果老年全髋置换患者术后隐性出血总量平均为1523 mL,隐性失血为576 mL,占总量的37.82%。B组比A组末次随访时SH-36评分、Harris评分高,差异有统计学意义(P<0.05)。结论隐性失血量多少对于老年全髋置换患者术后生存质量有一定影响,减少患者术后隐性失血量能有效促进患者术后功能康复,提高其生存质量。Objective：To investigate the relationship between hidden blood loss and quality of life after total hip arthroplasty in the elderly.Methods：87 patients were analysed and divided into two Groups.Group A,in which the HBL was higher than the average.Group B,in which the HBL was lower than the average.Statistical analyses were conducted to compare two groups in the SF score,Harris score preoperatively and at the time of the last follow-up.Results：The mean blood loss was 1523 mL and the hidden blood loss was 576 mL（38%）.The SH-36 score,Harris score at the time of the last follow-up in Group A significantly higher than Group B（P0.05）.Comclusion：Hidden blood loss maybe a useful prognostic information contributing to the functional rehabilitation and the quality of life in the elderly after THA.
