中西医结合治疗围绝经期子宫出血的机制研究

摘　　要：目的探讨中药联合妈富隆治疗围绝经期子宫出血的临床效果和治疗机制。方法将160例患者随机分为中药治疗组(自拟益气固本汤+妈富隆)80例和西药对照组(妈富隆)80例,1个月经周期为1疗程,观察3～6个疗程,对两组治疗前后的性激素、血红蛋白、凝血酶原时间、子宫内膜厚度及总有效率进行比较。结果中药治疗组总有效率97.50%高于西药治疗组的86.25%(P<0.05)。治疗后两组子宫内膜厚度较治疗前显著降低(P<0.01或0.05),中药治疗组降低更为显著(P<0.01)。中药治疗组治疗后FSH和LH明显降低,血清P和E2明显升高(P<0.05)。西药治疗组治疗后FSH和LH降低均不明显。血清P和E2明显升高,而且较中药组升高更明显(P<0.05);中药治疗组治疗后APTT及PT均较治疗前降低(P<0.05),而西药治疗组治疗后APTT及PT较治疗前降低均不明显。结论中西医结合治疗围绝经期功血效果显著,机制明确,可推广使用。Objective：To explore the curative effect of Chinese herb and marvelon on climacterium dysfunction uterine bleeding.Methods：160 women were randomly divided into 2 groups.One group with marvelon and Yiqiguben Decoction,while another group with single marvelon.All the patients were treated for 3 to 6 course of treatment.Hemoglobin,prothrombin time,the thickness of endometrium and the variations of endocrine were observed during the drug-administering period and after the course of treatment ended.Results：In the Chinese herb group the total efficiency of the patients was 97.50%,which was higher than that of western medicine group whose was 86.25%（P0.05）;The thickness of endometrium of two groups all decreased after treating.The Chinese herb group＇s curative effect of controlling endometrium hyperplasia was better than the the contrast group（P0.01）.P and E2increased and Hemoglobin were enhanced in two groups（P0.05）.FSH,LH,APTT and PT decreased only in the Chinese herb group（P0.05）.Conclusion：The combination of Chinese herb and Marvelon has the great effect on the climacterium dysfunction uterine bleeding.The clinical effects of combined Chinese herb and Marvelon were better than alone Marvelon.
