胸部结节病结节星系征的CT表现及治疗后变化

摘　　要：目的探讨胸部结节病结节星系征的CT影像学特征及治疗后改变。方法搜集并分析170例胸部结节病的CT影像,选取有结节星系征影像学表现的15例进行回顾性分析。结果 15例结节星系征共计病灶41个,右肺上叶9个(21.9%),中叶8个(19.5%),下叶5个(12.2%);左肺上叶8个(19.5%),左肺上叶舌段7个(17.1%),左肺下叶4个(9.7%)。所有结节星系征均为多发病变,未发现有孤立单发病变。直径1～2 cm的结节21个(51.2%),2～3 cm结节17个(41.5%),>3 cm者3个(7.3%)。15例结节星系征均合并肺内其他改变。合并胸内淋巴结增大12例(80%)。15例结节星系征41个病灶治疗后随访动态CT改变:完全吸收9个,部分吸收24个,无变化6个,进展2个。结论胸部结节病结节星系征CT表现是指肺内的由许多小结节融合的大结节,周围有众多的小结节样的卫星灶围绕,伴向中心融合趋势,其形态特征如同星系中数以万计的小星星聚集在一起围绕着大行星,是胸部结节病很有价值的影像学表现,治疗后吸收好转明显。Objective To investigate CT features of sarcoid galaxy sign in pulmonary sarcoidosis and changes after treatment. Methods Among 170 cases confirmed as pulmonary sarcoidosis, 15 cases showing sarcoid galaxy sign were retrospectively studied. Results A total of 41 lesions of sarcoid galaxy sign were identified in the 15 patients, 9 （21.9%） in the right upper lobes , 8（ 19.5% ） in the right middle lobes , 5（ 12.2% ） in the right lower lobes , 8（ 19.5% ） in the left upper lobes, 7 （ 17.1% ） in the left lingnla lobes, 4 （9.7%） in the left lower lobes. All sarcoid galaxy sign were multi- pie lesions. Among 41 lesions, the size of lesions were 1 - 2cm in diameter in 21 （51.2%） ,2 - 3cm in 17 （41.5%）, greater than 3cm in 3 （7.3%） lesions. All of the 15 cases were complicated with other lesions in the lungs, thickening of bronchovascular bundle in 10 （66.7%） patients , nodules and consolidation in 7 （46.7%）, ground glass shadow in 4 （26.7%）, interlobular septal line in 5 （33.3%）. Combined with hilar and mediastina lymphadenopathy in 12（80% ）. CT imaging changes in 15 patients of 41 lesions after treatment ： overall improvement in 9, improvement in 24, no change in 6, progression in 2. Conclusion The CT appearance of sarcoid galaxy sign showed as large nodules surrounded by many tiny satellite nodules. The sarcoid galaxy sign was a valuable imaging findings in pulmonary sarcoidosis, which can be im- proveed obviously after treatment.
