摘 要:目的探讨新双源CT Flash扫描模式下,对比剂优化注射方法在小儿先天性心脏病CT成像中的初步应用价值。方法 43例(分为A、B两组)复杂型先天性心脏病患儿,均接受新双源Flash CT(西门子炫速双源CT,德国)血管增强扫描。A组(18例)为Flash CT常规注药方式及扫描组,对比剂的用量及注射方式均采用常规用量及用法;B组(25例)为优化扫描组,对比剂的用量及注射方式均采用优化方案。对A、B两组间的图像质量、对比剂用量进行比较。结果两组间图像质量的评价参数一致,上腔静脉、右心房、右心室、肺动脉、左心房、左心室、升主动脉及降主动脉的增强CT值A组高于B组(P"0.05),心腔及大血管不同部位CT差值,两者之间比较差异有显著性统计学意义(P=0.00),其中B组上腔静脉、右心房的图像质量明显优于A组(P!0.05),而A组肺动脉、左心房、左心室、主动脉的图像质量明显优于B组(P!0.05)。A组的对比剂用量明显多于B组(t=2.78,P=0.038)。结论对比剂优化注射方法采用较少的对比剂剂量,可消除上腔静脉伪影提高图像质量,适用于复杂型先天性心脏病CT增强扫描,有望推广应用。Objective To explore clinical value of Somatom Definition Flash CT contrast injection optimization in CT angiography to display the congenital heart disease in children. Methods Forty three patients( derided into group A and group B)with complex CHD underwent Flash CT angiography. Group A (18 cases)was injected contrast agent with the con- ventional method and dose, group B (25 cases)with the optimized method and dose. The subjective and objective image quality, contrast agent doses in two groups were compared. Results The CT values of superior vena tara, right atrium, right ventricle, main pulmonary artery, left atrium, left ventricle, ascending and the descending aorta in the two groups had statistical significance ( P 〈 0.05 ). The image quality of superior vena tara, right atrium was significantly better in group B than that in group A( all of the P values were 〈 0.05 ). The image quality of pulmonary artery, left atrium, left ventriele, aorta was significantly better in group A than that in group B ( all of the P values were 〈 0.05 ). The mean contrast agent dose in group B was significantly lower than that in group A ( P = 0.038 ). Conclusion contrast injection optimization in computed tomography angiography eliminated the image artifacts of superior vena cava and improved the image quality with less contrast agent dose. It is expect to be widely applied for complex CHD.
