摘  要:目的建立幼鼠高位脊髓损伤神经源性膀胱(neurogenicbladder,NB)模型并对其进行尿动力学评估,为临床研究神经源性膀胱的病理生理提供一定参考。方法22只健康Wistar雌性大白鼠,体重(200±20)g,4~6周龄,随机分为NB组(n=14)和正常对照组(假手术组,n=8)。NB组采用改良的Allen’S法(冲击损伤法50gcm)建T12脊髓损伤模型。观察对照组与NB组术后546d神经损害的表现并进行影像尿动力学评估。结果NB组大白鼠术后出现后肢瘫痪等脊髓损伤表现,对照组(n=8)无神经损伤表现。和对照组相比影像尿动力示NB组逼尿肌无收缩,膀胱最大容量[-maximumbladdercapacity,MBC,(3.93±2.03)ml比(0.081±0.247)ml,P〈0.001]和膀胱顺应性(bladdercompliance,BC,0.093±0.54kg0.022±0.01,Pd0.001)显著增加,逼尿肌漏尿点压(1eakde—trusorpressur,LDP,45.38±12.52比26±3.85,P〈0.001)显著升高。结论采用改良的Alleys法(冲击损伤法50gcm)能建立幼鼠高位脊髓损伤NB模型,尿动力学检查符合NB改变。Objective To evaluate urodynamic study in young rats with neurogenic bladder (NB) induced by high-level spinal cord injury. Methods Twenty-two young female Wistar rats [-mean weight (200 8± 20) ,4-6 weeks old] were randomly divided into NB group (n = 14) and normal control group (n = 8). NB group was made using modified method of Allen's (Impact damage method 50 gcm) at the twelve thoracic spine level. Video-urodynamic assessment was performed at 5-6 days post inju- ry. Results NB group showed typical postoperative manifestations of hind limb paralysis. The control group showed no significant abnormality. Detrusor muscle showed areflexia, maximum bladder capaci- ty (MBC) and leak detrusor pressure (LDP) and bladder compliance (BC) increased significantly in NB group than those of control group [-MBC(3.93 ± 2. 03)ml vs. (0. 081 ±0. 247) ml; BC(0. 093 ± 0. 54)ml vs. (0. 022 ± 0.01)ml; LDP(45.38 +- 12. 52)ml vs. (26 ± 3.85)ml respectively] (P〈0. 001 ). Conclusions The modified method of Allen's is useful to establish NB.
