摘  要:目的探讨应用负载同种脂肪干细胞的异种真皮脱细胞基质生物复合材料修复兔尿道缺损的效果。方法将54只新西兰雄性大白兔随机均分三组,A组(实验组)应用负载脂肪干细胞的异种真皮脱细胞基质修复缺损兔尿道,B组(对照组Ⅰ)用真皮脱细胞基质材料进行修复,C组(对照组Ⅱ)将尿道断端直接缝合。于术后2、4、7、16周切取尿道标本,行组织学观察并比较各组问毛细血管及平滑肌数有无差异。术前术后行尿动力检查,比较最大尿道压有无差别。结果术后组织学观察A组可形成近似正常的尿道结构,B和c组尿道结构不如A组。术后7周时比较A、B、c三组新生尿道毛细血管数(15.8±2.51:E12.3±2.6比7.1±2.3)及平滑肌数(18.1±3.1比13.5±2.7比8.9±2.6),A组多于B、c组,且组间差异有统计学意义(P〈0.05)。A组术前术后最大尿道压无明显差别[(24.3±2.6)cmH20比(25.2±2.0)cmH20,P〉0.05],B组术前术后差别有统计学意义[(23.6±2.3)cmH:O比(28.7±2.0)cmH:O,P〈0.00。结论脂肪干细胞作为种子细胞可用于构建组织工程化尿道;脂肪干细胞复合脱细胞基质能加速新生尿道毛细血管及平滑肌再生,有助于减少术后并发症。Objective To evaluate the effect of urethra[ repair by hetero[ogous dermal ace][u!.ar matrix seeded with rabbit adipose derived stern cells (ADSCs). Methods Fifty-four rabbits were ran- dornly divided into three groups. Group A (experimental group) : heterologous dermal acellular matrix with ADSCs were used for repairing a 2 crn length of urethra] defect. Group B (control group I) : re- paired only with heterologous dermal acellular matrix. Group C (control group ID : urethral defect was sutured and repaired simply. The newly laid down urethral tissue was harvested regularly (2, 4, 7 and 16 weeks after surgery) for histological examination. The differences in blood vessels and smooth muscles were compared in the three groups. Uroclynamics was tested to compare the differences of rnaximum urethra pressure before and after surgery. Results Almost normal urethral tissue could be found in group A, and it was better than group B and group C. The number of blood vessels and smooth muscle was much more in group A than the other groups. There was significant difference a- mong them (18. 1 ± 3.1 vs 13. 5± 2. 7 vs 8.9 ±2. 6, P〈0. 05). The maximum urethra pressure in group A showed no significant difference after operation](24. 3± 2. 6) cmH20 vs (25. 2 ± 2. 0)crnH2 0,P〈0. 05], but there was significant difference in group B [(23. 6 ± 2. 3)cmH20 vs (28. 7 ± 2. 0) cmH20,P〈0. 01]. Conclusions ADSCs are feasible for the reconstruction of tissue-engineered ure- thra. Dermal acellular matrix compounded with ADSCs helps to reduce postoperative complications due to accelerated regeneration of new urethral blood vessels and smooth muscles.
