子宫内膜轻微吸刮术改善反复着床失败妇女妊娠结局的应用

摘　　要：目的:探讨子宫内膜微吸刮术对反复着床失败妇女再次体外受精-胚胎移植(IVF-ET)临床妊娠结局的影响。方法:选择2011年8月至2012年5月北京大学第三医院生殖医学中心的164例反复着床失败妇女进行前瞻性对照研究,研究组82例于再次IVF-ET术前行子宫内膜轻微吸刮术,对照组82例IVF-ET术前未行子宫内膜轻微吸刮术。观察两组患者妊娠结局。结果:两组患者年龄、不孕年限、不孕原因、基础FSH值比较,差异均无统计学意义(P>0.05)。两组间促性腺激素使用天数、HCG注射日子宫内膜厚度、获卵数、移植胚胎数比较,差异均无统计学意义(P>0.05)。研究组和对照组胚胎着床率(22.12% vs 10.55%)、临床妊娠率(41.46% vs 20.73%)及继续妊娠率(36.58% vs 15.85%)比较,差异有统计学意义(P<0.01)。结论:子宫内膜轻微吸刮术能提高反复着床失败妇女再次IVF-ET术后的临床妊娠率,改善临床妊娠结局。Objective：To explore the effect of endometrial scratching on outcomes of IVF-ET in women with repeated implantation failure（RIF）. Methods： From August 2011 to May 2012,164 women with RIF who planned to perform IVF-ET again were included. In study group（82 cases）endometrial scratching were con- ducted prior to IVF-ET,while in control group（82 cases） ,no endometrial scratching prior to IVF-ET. The out- comes of IVF-ET were compared between these two groups, Results：There were no significant differences between theses two groups in the age of the patients, duration and cause for infertility, basal level of FSH, re- sponse to hormone stimulation,the endometrial thickness on the day of HCG injection, numbers of retrieved oocytes,embryos transferred,and the way of fertilization （P 〉 0.05）. There were significant difference in em- bryo implantation rate,clinical pregnancy rate and ongoing pregnancy rate between these two groups,they were 22. 12% ,41.46% and 36.58% in study group respectively,and 10.55% ,20.73% and 15.85% in con- trol group（ P 〈0.01 ）. Conclusions： Endometrial scratching improves the clinical pregnancy rate and clinical pregnancy outcomes of IVF-ET in women with RIF.
