摘 要:目的:探索晚发型子痫前期患者血清差异蛋白表达的变化,为其早期预测妊娠期高血压疾病提供实验依据。方法:用表面增强激光解析电离飞行时间质谱(SELD-ITOF-MS)技术,检测15例晚发型子痫前期患者(晚发型子痫前期组)、15例正常孕妇(正常对照组)和15例未孕高血压患者(未孕高血压组)血清蛋白,并获得蛋白芯片表达图谱。用Biomarker Wizard软件和Biomarker Pattern软件分析并建立蛋白质指纹图谱模型。根据所建模型对晚发型子痫前期(21例)和正常妊娠妇女(47例)进行盲筛,并统计其诊断特性。结果:①在分子量2000~25000范围内,晚发型子痫前期组与正常对照组血清蛋白质指纹图谱之间有4个蛋白质峰,差异有统计学意义(P<0.05),晚发型子痫前期组与未孕高血压组血清蛋白质指纹图谱之间有16个蛋白质峰,差异有统计学意义(P<0.05)。②分析系统筛选出质荷比为5650.40 Da和13991.0 Da的标志蛋白建立妊娠期高血压疾病的诊断模型,应用该模型盲筛,其预测灵敏度为69.6%(16/23),特异度为85.1%(40/47),准确率为82.4%(56/68)。结论:通过SELD-ITOF-MS技术筛选出的血清差异蛋白可作为晚发型子痫前期发生、发展的特异性指标。本研究建立的分类决策树模型对妊娠期高血压疾病的早期预测及诊断有一定的意义。Objective:To explore the serum proteomic fingerprints of patients with Late-onset preeclamp- sia and try to provide experimental basis for the early prediction of gestational hypertension disease. Meth- ods:Fifteen patients with late-onset preeclampsia( late-onset preeclampsia group) and fifteen healthy preg- nant women (normal control group)and fifteen Hypertension (non-pregnant hypertension group)were tested by surface-enhanced laser desorption / ionization-time of flight-mass spectrometry (SELD-ITOF-MS). Bio- marker Wizard and Biomarker Pattern software were used to detect protein peaks which were significantly dif- ferent between HDCP and contro!s, and to establish a primary diagnostic model of HDCP. According to the model,late-onset preeclampsia (21 cases) and normal pregnant women (47 cases) were blind screening and the diagnosed characteristics was also analyzed. Results :(~Four protein peaks were significantly differ- ent at the molecular range of 2000 ~ 50000 between in gestational hypertension group and in control group (P 〈 0.05). Sixteen protein peaks were significantly different between in gestational hypertension group and control group ( P 〈 0.05 ). ~Two proteins molecules (5650. 40,13991.0 respectively) were screened as a proteomic model ,the predictive sensitivity was 69.6% (16/23), specific degrees was 85. 1% (40/47), ac- curacy was 82.4% (56/68). Conclusions: The differently expressed proteins in serum screened by SELD-ITOF-MS could act as the biomarkers of the pregnant hypertension's occurrence and development.The diagnostic pattern built in this study maybe useful for early diagnosis of HDCP.
