摘 要:目的探索建立犬月骨缺血性坏死动物实验模型的方法。方法成年健康杂种犬12只,通过剥离左侧月骨周围组织及液氮冷冻的方法致其缺血坏死,分别在术后第3、6、12周动态地观察其影像学(x线片、Cr及MRS)表现,并取月骨进行大体标本及组织形态学检查。结果第3周时X线片及cT检查没有明显的影像学改变,MRl检查T2WI上表现为弥漫性的点状高信号影。第6周时x线片及CT检查没有明显的影像学改变;MRI表现异常,12踟上表现为片状高信号影。第12周时x线片示腕骨骨密度不均匀,腕骨外形不规则,压缩变扁;CT示大面积不规则的低密度灶;MRI检查T2WI上见腕骨内部大面积的高信号影;此时大体标本见大片白色死骨,关节软骨脱落;光镜下见骨小梁密度减低,大量骨陷窝形成。结论运用液氮冷冻并血管破坏可以制备典型的犬月骨缺血性坏死动物实验模型。Objective To establish a canine model for the study of ischemie necrosis of the lunate. Methods Twelve adult canines were used. The lunate was exposed and stripped off its peripheral tissues and then frozen by liquid nitrogen. X-ray, CT and MR/images of the hmate were acquired at 3, 6 and 12 weeks to track the dynamic changes. Lunate was harvested at these time points for gross and histology examination to evaluate the necrosis of lunate and subsequent repairing reaction. Results At 3 weeks there were no significant changes on X-ray and CY. Diffused sporadic enhanced signals were seen on T2 weighted MRI images. At 6 weeks X-ray and CT scans were still insignificant. However T2 weighted MR/showed more abnormality with areas of enhanced signals. At 12 weeks hetemgenous bone density, irregularity and compression deformity of the carpal bone were seen on X-ray. CT scans showed large areas of low density loci, while MR/showed large areas of enhanced signals on T2 weighted images. There were significant changes in gross and microscopic mmfifestation of the lunate. Large area of whitish necrotic bone and decaying of cartilage surface were seen. Light microscopic sections revealed decreased bone density in the tralx^ulum and loci of bone collapse. Conclusion Liquid nitrogen freezing and devascularization of the lunate is a successful and reliable method to establish a canine model of Kienbock's disease.
