影响空心加压螺钉治疗股骨颈骨折骨愈合的因素分析

摘　　要：目的:分析应用空心加压螺钉治疗股骨颈骨折影响骨愈合的相关因素。方法:回顾性分析2004年1月-2011年1月采用空心加压螺钉手术治疗的股骨颈骨折患者。比较骨折愈合组与骨不愈合组患者的一般特点,分析影响骨愈合的相关因素。分析变量包括:性别、年龄、创伤类型、骨折类型、有无骨质疏松、是否急诊手术、骨折的复位质量、是否切开复位等。结果共有190例患者符合纳入标准,年龄在21～83岁,平均年龄为65岁,有23例出现骨不愈合,不愈合率为12.1%,总愈合率为87.9%。单因素各组间比较结果显示:年龄、骨折类型、创伤类型、骨折复位质量、骨质疏松等比较差异有统计学意义(P<0.05)。而在性别、急诊与择期手术、是否切开复位等比较差异无统计学意义(P>0.05)。结论:年龄、骨折类型、创伤类型、骨折复位质量、骨质疏松情况等因素均对空心加压螺钉治疗股骨颈骨折骨愈合疗效产生显著影响,避免影响骨愈合疗效的不利因素有助于改善术后骨愈合疗效。Objective：To analysis the factors of bone healing of patients with femoral neck fracture after treating with cannu- lated compression screw fixation. Methods： Retrospective analyzed the patients of femoral neck fractures which had been treated with cannulated compression screw fixation from January 2004 to January 2011. The general characteristics were compared between healing group and nonunion group, and the influential factors of bone healing were analyzed, including gender, age, type of trauma and fracture, with or without osteoporosis, whether the emergency operation, the fracture re- duction quality, whether open reduction and so on. Results：A total of 190 patients matched the selection criteria, age 21-- 83, and the average age was 65 years old. There were 23 cases with nonunion, the rate of nonunion was 12.1%, and total rate of healing was 87. 9%. Univariate analysis results showed that among the three groups： age, fracture and trauma type, fracture reduction quality, osteoporosis differences were statistically significant （P〈0.05）. In gender, emergency and elective surgery, whether open reduction was no significant difference （P〉0. 05）. Conclusions： Age, fracture and trauma type, fracture restoration quality, and osteoporosis factors are significant impact the bone healing in the patients with femoral neck fracture after treating with cannulated compression screw fixation. So to avoid the unfavorable factors can improve the effect of bone healing.
