摘 要:目的:验证自制胫骨机械轴定位器对提高膝骨关节炎TKA临床疗效的影响。方法:从2012年2月-2012年8月间对52例共67膝随机分成两组进行膝关节置换。试验组:应用自制胫骨机械轴定位器,在透视下术前确定胫骨机械轴并在小腿前侧皮肤做标示线。髓外定位系统术中按标识线引导进行胫骨截骨,完成手术。对照组:按常规胫骨髓外定位系统定位进行截骨完成手术。术后对胫骨假体力线准确率及术后2周、6个月膝关节HSS评分进行比较。结果:试验组胫骨假体力线准确率高于对照组(P<0.05),2周膝关节HSS评分试验组和对照组无差异(P>0.05),6个月膝关节HSS评分试验组平均分数优于对照组(P<0.05)。结论:自制胫骨机械轴定位器可以提高TKA中胫骨假体力线准确率及术后中远期疗效。Objective:To discuss the effect of the tibia mechanical axis positioner on TKA for osteoarthritis of knee. Meth- ods:52 cases of 67 knee joints were randomly divided into the experimental group and the control group. The patients of the experimental group underwent TKA using tibia mechcaical axis positioner. While the patients of the control group under- went TKA using intramedullary guide system. The differences of two groups in the precision of tibial component alignment and the HSS score were compared 2 weeks and 6 months later. Results:The experimental group was superior to the control group in the precision of tibial component alignment and the HSS score 6 months later. While the experimental group had no difference with the control group in the HSS score 2 weeks later. Conclusion: The application of the tibia mechanical axis positioner on TKA is beneficial for the tibial component alignment and long-term effectiveness.
