手法复位外固定支架固定术治疗盖氏骨折的临床观察

摘　　要：目的:在继承传统手法复位的基础上,总结外固定支架固定治疗盖氏骨折的经验,观察其临床疗效,初步探讨其作用机理。方法:搜集90例病例,30例为治疗组,60例为对照组。治疗组通过手法复位外固定支架固定术进行治疗;对照组分为2组,分别通过传统手法复位小夹板外固定(对照组Ⅰ)、和西医常规手术(对照组Ⅱ)进行治疗。所有病例根据Cooney腕关节评分系统分别在治疗后第3天、2周、8周、12周、24周进行评分并比较。治疗后8周参照国家中医药管理局1994年制定的《中医病证诊断疗效标准》评定标准进行疗效评定。结果:治疗组治愈率及总有效率与对照组Ⅰ和对照组Ⅱ比较差异有统计学意义;治疗后第3天、第2周、第8周、第12周及第24周,治疗组和对照组Ⅰ、对照组Ⅱ在症状缓解上比较差异有统计学意义。结论:手法复位外固定支架固定术可以有效的治疗盖氏骨折,并且经过治疗后不论是短期还是远期观察来看,患者的症状均得到明显改善。Objective：To observe the curative effect of bone-setting combined with external fixation for treating Galeazzi frac- ture. Methods： All 90 cases of Galeazzi fracture were randomly divided into treatment group, control group Ⅰ , and control group Ⅱ The patients in the treatment group were treated by bone-setting and external fixation. The patients in the control group Ⅰ were treated by the conservative treatment of TCM. The patients in the control group Ⅱ were treated by the opera- tion of western medicine. The Cooney scoring system for wrist was used to assess 3 days, 2 weeks, 8 weeks, 12 weeks, and 24 weeks after treating. And the clinical effective rate was evaluated 8 weeks after treating. Results：There was signifi- cant difference in the rate of cure and total efficiency between the treatment group and the control group. There was signifi- cant difference on improving the patients symptom between the treatment group and the control group 3 days, 2 weeks, 8 weeks, 12 weeks, and 24 weeks after treating. Conclusion：The bone-setting combined with external fixation is effective for Galeazzi fracture, and the patients＇ symptom can be improving obviously both in the short-term and long-term.
