摘 要:目的探讨具有中枢性调控作用的颈交感神经阻滞对严重烧伤大鼠的治疗作用,明确颈交感神经阻滞对严重烧伤大鼠胃肠运动功能及结肠平滑肌细胞表型转化的影响。方法将体重约200 g的SD大鼠随机分成正常组,烧伤对照组和CSB组,烧伤组和CSB组均制作成30%TBSAⅢ度烧伤模型。CSB组通过颈交感神经双侧连续阻滞6 d,正常组和对照组颈部连续注射生理盐水6 d。通过免疫组化观察大鼠结肠平滑肌α-肌动蛋白表达情况。结果在实验后的第6天,与正常组(76.23±3.82)%相比,烧伤对照组的炭末推进率(61.14±3.71)%显著降低,而CSB组(73.85±4.10)%无明显变化,但是CSB组炭末推进率要明显高于烧伤对照组。在烧伤对照组(26.75±3.68)中,大鼠结肠平滑肌细胞α-肌动蛋白表达水平要显著低于正常组(35.13±5.27)和CSB组(32.08±4.53),正常组和CSB组没有明显差异。结论大鼠的胃肠运动功能在烧伤后明显受到了抑制,通过颈交感神经阻滞能够显著改善这种抑制作用。具有中枢性调控作用的颈交感神经阻滞能够提高结肠肌α-肌动蛋白表达,增强结肠平滑肌的收缩功能,这将有利于肠道运动功能的维持,防止肠道缺血,防止细菌的过度繁殖及移位,促进机体的修复。Objective To study the effects of cervical sympathetic block(CSB) as central regulation for the therapy of severe burned rats, explore the roles of cervical sympathetic block in gastrointestinal motility and colonic muscle cell phenotype switch in severe burned rats. Methods SD rats with 200 g body weight were randomly divided into Normal, Burned and CSB group. Both Burned and CSB groups were constructed 30% TBS Ⅲ burn injured mod- el. Two sides cervical sympathetic block was adopted for 6 days in CSB group. The normal group and control group were injected with saline for 6 days. Following, observe the expression of ot - actin via immunohistochemisry in all groups. Results In the first six days after the experiment, compared with the normal group (76. 23% ± 3.82% ), carbo medicinalis propulsive rate of burned control group(61.14% ± 3.71% ) significantly reduced, while no signifi- cant change with the CSB group(73. 85% ±4. 10% ). But the carbo medicinalis propulsive rate of CSB group is sig-nificantly higher than the burned control. In the burned control group (26.75 ± 3.68 ), the expression of alpha - smooth muscle actin in the smooth muscle cells of the rat colon is significantly lower than the normal group(35.13 ± 5.27) and CSB group(32. 08 ±4. 53), but there is no significant difference between normal group and CSB group. Conclusion The inhibition of gastrointestinal motility in severe burned rats is significantly improved after cervical sympathetic block. Cervical sympathetic block as central regulation can improve colon muscle α -smooth muscle ac-tin expression which will enhance colonic smooth muscle contraction, benefit the intestinal absorption to prevent bac-terial overgrowth and migration, promote the burn repaired.
