甲状腺功能亢进患者的糖耐量分析

摘　　要：目的分析甲状腺功能亢进患者体内的的糖耐量异常以及胰岛素抵抗的情况。方法选取我院内分泌科收治的60例甲状腺功能亢进症患者,选取同期行健康体检的健康受试者为对照组,对两组受试者行口服葡萄糖耐量试验,比较糖耐量以及胰岛素抵抗情况。甲亢组糖耐量受损患者经过治疗后,比较治疗前后的FT3、FT4以及糖耐量和胰岛素抵抗的情况。结果而甲亢组餐后2 h血糖,餐后2 h胰岛素水平明显高于健康对照组(p<0.05);两组受试者的ISI及HOMAIR差异有统计学意义(p<0.05)。甲亢组糖耐量受损患者经过治疗后,其血清游离三碘甲腺原氨酸以及血清游离甲状腺素,餐后2小时血糖、餐后2小时胰岛素以及HOMA-IR较治疗前明显下降,而ISI以及HOMA-IS明显上升,差异有统计学意义(p<0.05)。结论在治疗甲状腺功能亢进症时,要注意其体内糖代谢异常,明确其糖耐量受损的情况,并实施针对性治疗以提高疗效。Objective To analyze the abnormal glucose tolerance and insulin resistance of patient with hyperthyroidism. Methods 60 patients with hyperthyroidism were selected in our hospital, the healthy subjects received health examination were selected as the control group, the glucose tolerance and insulin resistance of both groups were compared. The indexes of FT3, FT4, glucose toler-ance and insulin resistance of the patients before and after treatment were compared. Results The 2 hours postprandial glucose and 2 hour postprandial insulin level of the hyperthyroidism patients were significantly higher than that of contrel group （p 〈 0. 05 ） ; the ISI and HOMA -IR of both groups showed significant difference（p 〈 0.05 ）. After treatment, the concentration of serum three free io-dine, thyroid gland original acid, the serum free thyroxine, 2 hour postprandial blood glucose, 2 hour postprandial insulin and HOMA -IR decreased significantly in the patients with hyperthyroidism, while the ISI and HOMA-IS increased significanfly（p 〈 0. 05 ）. Conclusion Much attention should be paid to the abnormal glucose metabolism in the treatment of patients with hyperthyroidism to declare the situation of damage of glucose tolerance and implement pertinency therapy in order to improve the clinical efficacy.
