南海区临床用血合理性控制体系的建立和应用评价

摘　　要：目的为了解南海区临床用血合理性情况，提高临床科学合理用血水平，建立和应用行之有效的“临床用血合理性控制体系”。方法2009—2012年每年开展临床用血督导检查，抽查临床用血病历，分析其用血的合理性；自2011年起建立并应用“南海区临床用血合理性控制体系”；统计分析2009—2012年期间每年红细胞类、血浆类血液成分不合理使用的比例。结果2011年和2012年各年红细胞类、血浆类使用的不合理率均比2009年或2010年的明显下降，均具有显著性差异（均P〈0．05），且不合理率逐年下降，临床不合理用血情况得到有效改善。结论“南海区临床用血合理性控制体系”科学、有效，实现了卫生局、血站、医院“三方联动”联合管理临床合理用血工作的立体模式，应持续应用并不断完善。Objective In order to understand the rationality condition and enhance its rationality level of clinical blood transfusion in Nanhai district and to establish an effective rationality control system on the clinical blood transfusion. Methods Yearly conducted survey on supervision of the clinical blood transfusion and undergone random investigation on clinical blood transfusion cases and analyzed the rationality of blood transfusion from 2009 to 2012. From 2011, the Nanhai Rationality Control System of Blood Transfusion was established and put into force. Statistical analysis on the rate of irrational use of red blood cell and plasma was carried out yearly from 2009 to 2012. Results The rates of irrational use of red blood cell and plasma in year 2011 and 2012 are both significantly lower than that in 2009 or 2010. The differences both look significant（p 〈 0. 05 ） and the irrational rate decreased year by year. The ir- rational use of blood transfusion was effectively controlled. Conclusion The establishment of Nanhai Rationality Control System of Clinical Blood Transfusion is scientific and the application effect is obvious. A three - dimension pattern of combined management by Health Bureau, Blood Station and Hospital was realized and should be carried on and improved.
