摘　　要：目的：探讨扩髓交锁髓内钉治疗股骨干骨折方法及临床疗效。方法：回顾性分析83例股骨干骨折患者,按照Winquist分型：Ⅰ型13例,Ⅱ型18例,Ⅲ型以上52例,均采用交锁髓内钉扩髓内固定。结果：术后随访83例患者骨折全部骨愈合,膝关节功能良好。随访时间10~36个月,临床愈合时间4~6个月。无感染、无主钉锁定断裂等并发症。结论：根据骨折分型闭合或有限切开复位扩髓交锁髓内钉固定可以提高股骨干骨折疗效,必要的自体髂骨植骨术及术后适当功能康复锻炼,可明显提高骨愈合率,减少术后并发症,提高患者生活质量。Objective： To explore the clinical effect and method of femural fracture fixed with interlocking intramedullary nail after limitedly reamed marrow. Method： 83 patients of femural fractures were studied after fixed with interlocking intramedullary nails and limitedly reamed marrow, according to Winquist clinical type （ I 13 cases , II 18 cases , m and others 52 eases ） . Result： 83 patients were followed up 10~36 months, shown fracture bone healed and fine function of their knees.The bone healing time was 4-6 months .All cases had no infection, The implant had no breakage and other complications. Conclusion： The clinical effect of femural fracture patients fixed is good , with interlocking intramedullary nail after limitedly reaming marrow and reduction or limited open reduction according to clinical types. Bone grafting with iliae bone and function rehabilitation exercise can promote the bone healing and the quality of life, and decrease postopen complications.
