血清孕酮检测在早期先兆流产中的临床应用

摘　　要：目的探讨早期先兆流产中孕妇血清孕酮(P)检测的临床价值。方法选择2009年1月至2011年1月经自然受孕确诊为早孕的100例孕妇为研究对象,根据其妊娠结局分为正常宫内早孕组(n=34),先兆流产组(n=54),难免流产组(n=12)(本研究遵循的程序符合本院人体试验委员会所制定的伦理学标准,得到该委员会批准,并征得受试对象本人的知情同意,与之签署临床研究知情同意书)。3组孕妇年龄、孕产史、身高、体重、停经时间等方面比较,差异无统计学意义(P<0.05)。采集3组孕妇肘静脉血检测血清P值。结果 3组孕妇血清P值两两比较,差异均有统计学意义(P<0.01)。先兆流产组孕妇治疗后血清P值与正常宫内早孕组比较,差异无统计学意义(P>0.05),该组经治疗后保胎成功率达85.2%(46/54)。血清P值为30nmol/L可作为判断早期流产治疗成功与否的临界参考值。结论血清P可作为判定先兆流产预后的参考指标,以避免盲目保胎,提高保胎治疗成功率。Objective To investigate the clinical value of serum progesterone detection in the early threatened abortion. Methods From Januray 2009 to 2011,100 pregnant women who conceived naturally were included in the study,and divided into incipient abortion group（n= 54）, inevitable abortion group（n=12） and normal intrauterine pregnancy group （n = 34）. Serum progesterone levels were detected in three groups. The study protocol was approved by the Ethical Review Board of Investigation in Human Being of The Affiliated People＇s Hospital of Inner Mongolia Medical College. Informed consent was obtained from each patients. There were no significant differenc in general clinical data among three groups（P〉0.05）. Results There had significant differences in serum progesterone level among three groups（P〈0.01）. The serum progesterone level in incipient abortion group after treatment was not much higher than that of normal intrauterine pregnancy group（P〉0.05）. 30 nmol/L was a critical value of serum progesterone to determine the success of predicting tocolyis therapy. Conclusions Serum progesterone is a prognostic indicator of incipient abortion, and it can be used to decrease unnecessary tocolyis.
