摘  要:目的:观察分析不同分期非小细胞肺癌(NSCLC)患者血液屈服应力(τo)和甲襞微循环中白色微栓(WMT)的变化特点和临床意义。方法:102例NSCLC,包括肺鳞癌80例、肺腺癌22例;TNM分期:Ⅰ期15例、Ⅱ期42例、Ⅲ期30例、Ⅳ期15例。采用椎板式粘度计和微循环检查仪平行观测每例患者的τo水平和WMT计数,并进行各期均值比较及与正常人群比较。结果:Ⅳ期NSCLC患者的τo与对照组比较未见统计学差异(P>0.05),Ⅰ期、Ⅱ期和Ⅲ期患者τo显著高于对照组和Ⅳ期患者(P<0.05,P<0.01),Ⅱ期、Ⅲ期患者τo较Ⅰ期也显著升高(P<0.01)。各期NSCLC患者WMT计数明显多于对照组,且分期越高,WMT越多(P<0.05,P<0.01)。肺鳞癌与肺腺癌患者τo水平和WMT计数比较无统计学差异(P>0.05)。结论:Ⅰ期、Ⅱ期和Ⅲ期肺癌患者存在较严重血细胞缗钱体聚集,病情重者血栓性事件的风险较大。Objective: To observe characteristic and clinical significative of the different staging of nonsmall cell lung cancer (NSCLC) patients blood yield stress (τo) and white micro thrombus (WMT) in nailfold microcirculation. Method: 102 cases of NSCLC including 80 squamous cell carcinoma and 22 cases of lung adenocavcinoma; TNM staging, 15 cases in stage Ⅰ, 42 cases of stage II, 30 cases in stage III, 15 cases in stage IV by using vertebral plate viscometer and microcirculation examination instrument parallel observe each patient's τo and WMT,the measured value was compared with the normal population. Results: Stage IV patients τo compared with the control group no significant difference (P>0.05), stage II and III patients τo was significantly higher than that control group and stage IV patients(P<0.05,P<0.01). All patients WMT count were higher than the contrl group(P<0.05, P<0.01). There were no statistical differences in lung squamous cell carcinoma and adenocarcinoma of the lung cancer patients τo and WMT(P>0.05). Conclusion: In stage II and stage III of lung cancer patients had serious blood cells such as rouleaun type aggregation, greater risk of thrombotic event.
