摘  要:目的:观测心肌缺血后大鼠相关经穴和非相关经穴穴区皮肤血流量变化及高频和低频电针对其的影响。方法:将Wistar大鼠随机分为5组,正常组(6只,不作任何处理);假手术组(6只,只穿线不结扎);采用穿线结扎法制备心肌缺血大鼠模型(18只),6只留作模型组,另12只分别进行高频电针刺激(高频电针组,6只)和低频电针刺激(低频电针组,6只),电针穴位分别为双前肢"内关",电针时间20min。采用激光多普勒血流灌注成像仪分别于手术前和手术后10min检测正常组及假手术组、模型组,电针后即刻、10min、30min检测高频电针组和低频电针组相关经穴左"内关"、"郄门"和非相关经穴右"足三里"穴区皮肤血流量,分析高频电针组和低频电针组电针后不同时间各穴区血流量变化及与正常组、假手术组和模型组的差异。结果:模型组"内关"穴区皮肤血流量较假手术组和正常组明显增加(P<0.01),"足三里"穴区皮肤血流量较假手术组和正常组明显降低(P<0.01);与模型组比较,高频电针组和低频电针组电针后不同时间"内关"穴区皮肤血流量分别显著下降,尤以低频电针组下降更明显(P<0.01);"足三里"穴区皮肤血流量以低频电针组电针后10min显著增加(P<0.05)。各组间"郄门"穴区皮肤血流量无显著差异(P>0.05)。结论:心肌缺血模型大鼠"内关"穴区皮肤血流量较高,"足三里"穴区皮肤血流量较低,低频电针刺激"内关"穴可明显降低"内关"穴区和升高"足三里"穴区皮肤血流量,可能对缺血心肌有一定保护作用。Objective: To observe the change of skin blood flow between the related and non-related acupoints in myocardial ischemia rats, and the effect by high and low frequency electroacupuncture (EA) of 'Neiguan' (PC 6). Method: Wistar rats were randomized into 5 groups, normal control group(n=6, not be dealed with), sham-operation group(n=6, just be threaded not ligationed); myocardial ischemia rat model(n=18) was made with ligation by thread, 6 of which were included in the model group, the rest 12 were respectively intervened with high frequency EA(high frequency EA group, n=6) and low frequency EA(low frequency EA group, n=6), the acupoints by EA were PC 6 in rat two anterior lambs, the EA time was 20min. Skin blood flow of the left related acupoint PC6, 'ximen' (PC4) and the right non-related acupoint 'zusanli'(ST36) were detected by perfusion imager (FIM) before operation, 10min after operation in the normal control group and shamoperation, model groups respectively, instantly/10min/30min after EA in the low frequency EA and the high frequency EA groups. The change of these acupoints skin blood flow in different time after EA were detected. Results: The skin blood flow of PC6 in the model group was increased significantly compared with the sham-operation group and the normal control group(P<0.01), while decreased significantly in the skin blood flow of acupoint ST36 compared with the shamoperation group and the normal control group(P<0.01); in comparison with the model group, the skin blood flow of PC6 in the high frequency EA and low frequency EA groups both decreased, and effect of the low frequency EA was superior(P<0.01). In addition, the skin blood flow of ST36 was increased 10min after low frequency EA(P<0.05). The skin blood flow of PC4 in all groups were not significantly different (P>0.05). Conclusion: After myocardial ischemia, the skin blood flow of the rat left related acupoint PC6 increased significantly, and the skin blood flow of the right nonrelated acupoint ST36 decreased significantly.
