摘　　要：【目的】探讨后路经椎管椎体内植骨治疗胸腰椎不稳定性爆裂型骨折的疗效及安全性。【方法】本院胸、腰椎爆裂型骨折患者51例，采用后路经椎管椎体内植骨手术治疗，使用脊髓损伤评分（ASIA）、手术前后CT影像学比较、观察术后症状改善，骨折复位情况及有无术后不良事件，评价疗效。【结果】所有患者术后1个月内腰痛症状基本消失，AsIA评分较术前有所改善（P〈0．05），伤椎前、后缘高度丢失，后凸畸形cobb角，椎管侵占率均较术前明显纠正（P〈O．05），术后未出现严重并发症。【结论】后路经椎管椎体内植骨治疗胸腰椎爆裂型骨折，既能充分植骨，又能较好的重建椎管形态、恢复脊柱稳定性和消除对脊髓和神经的压迫，缓解患者症状，为功能恢复创造了良好的条件。[Objective] To explore the efficacy and safety of posterior vertebral body graft via spinal canal for the treatment of thoracolumbar unstable burst fracture. [Methods] A total of 51 patients with thoracolum- bar burst fracture in our hospital were treated with posterior vertebral body bone graft via spinal canal. Spinal cord injury score（ASIA） was used. CT images were compared between before and after operation. The im- provement of symptoms, fracture reduction and adverse events after operation were observed. The efficacy was evaluated. [Results]Postoperative symptoms of low back pain of all patients disappeared one month after oper- ation. Compared with before operation, ASIA score was improved（ P 〈0.05）. Compared with before treat- ment, height loss of anterior and posterior edge of diseased vertebra, kyphosis Cobb angle and vertebral canal encroachment rate were obviously corrected （ P 〈 0. 05）. No serious complication after operation occurred. [Conclusion]Posterior vertebral body graft via spinal canal for the treatment of thoracolumbar burst fracture can not only complete the bone graft fully, but also better reconstruct the spinal canal morphology, restore the stability of spine, eliminate spinal cord and nerve compression, alleviate symptoms and provide good conditions for the recovery of function.
