不同剂量舒芬太尼复合依托咪酯乳剂静脉诱导用于老年患者喉罩插入的临床效果

摘　　要：目的分析不同剂量舒芬太尼复合依托咪酯乳剂静脉诱导在老年患者喉罩插入的效果，探究最佳舒芬太尼剂量。方法选择喉罩全麻下术老年患者150例，患者年龄在65岁以上，ASAI或II级，随机分为5组各30例。插入喉罩前， I组：静脉注射0.05ug/kg舒芬太尼；II组：0.1ug/kg；III组：0.15ug/kg；IV组：0.2ug/kg；V组：0.25ug/kg；再对患者静脉注射依托咪酯乳剂0.2mg/kg。如果患者出现咳呛、吞咽、屏气、喉痉挛等反应则为不满意，反之表示麻醉满意。结果 I～V组满意例数分别为：0例、4例、14例、24例、30例。舒芬太尼ED50为0.15 ug/kg，95%CI为0.14～0.17 ug/kg。ED95为0.23 ug/kg，95%CI为0.2～0.28 ug/kg。结论老年喉罩插入患者中使用0.23 ug/kg舒芬太尼复合0.2mg/kg依托咪酯乳剂，患者满意度最好。同时其对患者血流动力学影响也非常小，有效地提高手术效果。Objective to analyze different doses sufentanil composite etomidate emulsion intravenous induction in elderly patients with laryngeal mask the effect of the insert, explore the best dosage sufentanil. Methods laryngeal mask surgery under general anesthesia, 150 cases of elderly patients and patients aged 65 or older, ASAI or class II, were randomly divided into 5 groups （n=30）. Before inserting laryngeal mask, group I：0.05 ug/kg intravenous sufentanil;Group II：0.1 ug/kg;Group III：0.15 ug/kg;IV group：0.2 ug/kg;V group：0.25 ug/kg;To patients with intravenous etomidate emulsion of 0.2 mg/kg. If patients cough reactions such as choking, swallowing, breathless, laryngospasm is not satisifed, and vice said anesthesia satisfaction. Results I^V group satisifed cases are：0 cases, 4 cases, 14 cases, 24 cases, 30 cases. Sufentanil ED50 0.15 ug/kg, 95%C1 from 0.14 to 0.14 ug/kg. ED95 0.23 ug/kg, 95%C1 from 0.2 to 0.2 ug/kg. Conclusion in elderly patients with laryngeal mask insert used in 0.23 ug/kg composite sufentanil 0.2 mg/kg, patient satisfaction is best. At the same time its inlfuence on hemodynamics in patients with also very small, effectively improve the effect of surgery.
