摘　　要：目的探讨2μm激光同期治疗前列腺增生并发膀胱结石的临床疗效。方法回顾性分析2011年3月-2012年6月本院完成的22例经尿道2μm激光同期治疗前列腺增生并发膀胱结石患者的临床资料。结果全部患者手术均获成功。患者中位年龄72（57~78）岁,结石中位直径12（3~20）mm,前列腺中位体积42.45（28.05~92.95）ml。2μm激光碎石中位时间28（9~46）min,手术中位时间100（40-130）min,术中中位出血量70（20-130）ml,术后留置尿管中位时间6（3~7）d。术后随访3~12个月,国际前列腺症状评分（international prostatic symptoms score,IPSS）由术前中位评分22（14~31）分降低到术后7（3~11）分（t=-12.591,P〈0.01）,最大尿流率（Qmax）由术前中位最大尿流率4.0（2.0~10.0）ml/s升高到术后19.5（15.0~24.0）ml/s（t=-14.111,P〈0.01）。结论采用2μm激光腔内同期治疗前列腺增生伴膀胱结石安全有效,是高龄、高危患者较为理想的微侵袭性手术治疗方法。Objective To study the clinical effect of transurethral 2μm continuous wave laser on prostatic hyperplasia with bladder stones.Methods Clinical data about 22 patients with prostatic hyperplasia accompanying bladder stones who underwent transurethral 2μm continuous wave laser operation in our hospital from March 2011 to June 2012 were retrospectively analyzed.Results The operation was successful in all the patients.The average age of the patients was 72 years（57-78 years）.The average diameter of stones was 12 mm（3-20 mm）.The average volume of prostates was 42.45 ml（28.05-92.95 ml）.The average time of lithotrity with 2μm continuous wave laser was 28 min（9-46 min）,the average operation time was 100 min（40-130 min）,and the average time of indwelling catheter was 6 days（3-7 days） after operation.The patients were followed up for 3-12 months after operation.The average international prostatic symptoms score（IPSS） decreased from 22（14-31） before operation to 7（3-11） after operation（t=-12.591,P0.01）.The Qmax increased from 4.0 ml/s（2.0-10.0 ml/s） before operation to 19.5 ml/s（15.0-24.0 ml/s） after operation（t=-14.111,P0.01）.Conclusion Transurethral 2μm continuous wave laser lithotrity is a safe and effective procedure for elderly and highrisk patients with prostatic hyperplasia accompanying bladder stones.
