摘　　要：目的探讨Rüedi-AllgowerⅡ～Ⅲ型Pilon骨折治疗方法、手术时机及并发症的防治。方法 2009年1月～2010年8月对12例Ⅱ～Ⅲ型Pilon骨折采取切开复位钢板内固定,7例辅以外固定,5例予以植骨修补骨缺损,其中自体松质骨填塞3例,大块髂骨植入2例。结果采用Burwell-Charnley系统进行骨折复位评分:优9例,良3例。踝关节功能Mazur评分:优8例,良3例,可1例。结论治疗Ⅱ～Ⅲ型Pilon骨折时,大块髂骨植骨能有效解决严重骨缺损、提高内固定稳定性,使关节早期活动,有利功能恢复。Objective To discuss and analyze the treatment of Pilon fractures（Rüedi-Allgower typeⅡ~Ⅲ）.Methods 12 patients with Rüedi-Allgower typeⅡand Ⅲ Pilon fractures were treated by open reduction and internal fixation,of whom 7 were assisted with external fixation and 5 were treated by bone transplantation（spongy bone=3,big pieces of ilium=2）.Results The restoration of fractures between January 2009 and August 2010 was evaluated with Burwell-Charnley system（excellent=9,good=3）;the function of ankle joint was evaluated with Mazur system（excellent=8,good=3,fair=1）.Conclusion In the treatment of Rüedi-Allgower typeⅡand Ⅲ Pilon fractures,the bone transplantation with big pieces of ilium is effective in solving the serious bone loss,strengthening the internal fixation and promoting earlier recovery of ankle joint.
