摘　　要：目的探讨"西部贫困家庭疝气儿童手术康复计划"的实施方案与管理。方法采用腹腔镜与开放疝囊高位结扎手术治疗儿童腹股沟疝并制定符合新疆地区特色的手术筛查方案,应用李嘉诚基金会、民政慈善基金、新农合基金免费治疗患儿。采取费用单病种控制,单侧腹股沟疝3500元,双侧腹股沟疝4000元封顶。结果成功完成219例患儿手术,手术时间5～20min,平均14min。术后住院天数平均1.3d。本组患儿者术后无围手术期死亡,无围手术期并发症发生,无复发。全部患儿费用均有效控制,未发生基金拒付事件。结论应用李嘉诚基金会、民政慈善基金、新农合基金免费治疗腹股沟疝患儿是国家的一项慈善事业,医疗机构在合理的成本核算与严格围手术期管理,有针对性处理围手术期的各种问题,是安全可行的,并成功建立单病种控制与临床路径,为我国相关疾病的慈善救助提供了有益的尝试与参考。Objective To investigate the ＂Rehabilitation program of children with hernia surgery from poor families in the West of China and its implementation and management. Methods The ligation laparoscopic and open hernia surgery and treatment of children with inguinal hernia surgery that has Xinjiang characteristics were developed in line with screening programs. The Li Ka Shing Foundation, the Home Charitable Foundation, and the new rural cooperative fund free treatment in children were applied. The single disease conu＇ol was taken including maximum 3,500 yuans for unilateral inguinal hernia and maximum 4,000 yuans for bilateral inguinal hernia. Results A total of 219 cases in children with surgery, postoperative patients in this group had no perioperative deaths, perioperative complications, and any recurrence. All patients were effectively controlled with costs, No payment was found for the cases. Conclusions Applications of Cazenove Foundation, Civic Foundation, the new rural cooperative fund free treatment of inguinal hernia in children is a national charity. A reasonable cost accounting in medical institutions, strict perioperative management, targeted treatment of perioperative a variety of issues, is feasible and safe. The single disease control and clinical pathways can be successfully established and can be positive reference for charity cares.
