深筋膜下封闭式负压引流术在切口脂肪液化裂开中的临床应用

摘　　要：目的探讨在深筋膜下应用封闭式负压引流术(VSD)对切口脂肪液化裂开再缝合后愈合的意义。方法回顾性分析河北省涿州市医院收治的切口脂肪液化裂开患者138例的临床资料。其中2008年10月至2010年10月,单纯行二期清创缝合75例(对照组)。2010年10月至2012年10月在二期缝合的基础上,在深筋膜下行VSD术63例(观察组),用中央吸引器对切口持续负压吸引5～7d,观察二组患者的愈合情况。结果观察组在切口继发感染和再次裂开二种症状上都明显优于对照组,差异有统计学意义(χ2=7.73、13.07,P=0.0054、0.0002)。结论在深筋膜下使用VSD技术,负压吸引对于切口脂肪液化裂开的术后处理有积极意义,而且方法简便易行,缩短愈合时间。Objective To discuss application of vacuum drainage canal under scarpa fascia in treatment cut split caused by fat liquefaction. Methods From October 2008 to October 2012, a total of 138 cases with cut split after surgery caused by fat liquefaction in the department of general surgery of Zhuozhou City Hospital were collected for analyses. The control group included 75 cases of ]I debridement suture and the observation group included 63 cases of deep fascia dowrdink VSD surgery based on the II suture. The incisions were handled with continuous negative pressure suction aspirator for 5 to 7 days to observe the healing of ihe patients in the two groups. Results In the observation group, the symptoms of the secondary infection and incision were significantly better than that of the control group with statistically significant difference （X2 = 7.73,13. 07, P = 0. 005 4,0. 000 2 ）. Conclusions By using the VSD technique under the deep fascia, negative pressure suction has positive significance for incision caused by fat liquefaction. The method is simple and convenient with shorten healing time.
