先天性小腿环状缩窄合并足畸形的治疗

摘　　要：目的 :对我院近 10年来治疗先天性小腿环状缩窄合并足畸形的手术方法和疗效进行总结。方法 :小腿环状缩窄的矫治分二阶段进行 ,第一阶段的手术时间为生后 2～ 7个月 ,施行一期环状缩窄切除及多个“Z”形转移皮瓣成形术 ,第二阶段的手术主要是矫治足的畸形 ,手术时间为 2～ 6岁 ,施行跟腱延长、Turco手术和石膏矫形。结果 :本组患儿均获得随访 ,马蹄内翻足、足内翻和扁平足各 1例随访时已痊愈 ,足循环障碍在第一阶段手术后已恢复循环。除先天性截肢外其余患儿均不影响足功能和足外形。结论 :小腿环状缩窄合并足畸形应争取早期施行环状缩窄切除术 ,以解除压迫在缩窄环下的主要外周神经 ,有利于足畸形的早期自愈。如果第一阶段术后足畸形好转不明显 ,应及早行第二阶段手术 ,以免患儿站立行走后足畸形加重。Objective: To summarize the operative methods and clinical effect of the congenital cnemis constriction band incoorporated with foot deformity cured in this hospital recent decades.Methods:The operative method was divided into two stages,the first-stage lysis and multitransfer skin flap operation finished in 2～7 months after the baby was born.The secondary operation was correction of foot malformation at 2～6 years old,including Achilles tendon elongation.Turco's operation and plaster orthopedic treatment.Results:All 8 patients were followed up.Equinovarus,flatfoot,pesvalgus were cured.Pedodysemia recuperated after lysis.Except two cases of contour.Conclusions:The patient should have been treated with constriction band lysis as maybe,so that the peripheral nerve can be released from constriction band.If the malformed foot improved unremarkably the secondary stage operation should be performed earlier,in case malformed foot becoming exacerbated after arise and walking.
