低分子肝素联合酚妥拉明治疗肺心病顽固性心衰的疗效观察

摘　　要：目的观察低分子肝素联合酚妥拉明治疗肺心病顽固性心衰的疗效。方法选择我院2010年5月至2012年11月收治的70例肺心病顽固性心衰患者，随机分为观察组和对照组各35例，对照组常规综合治疗，观察组在常规综合治疗基础上加用低分子肝素联合酚妥拉明治疗，比较分析两组疗效。结果观察组治疗总有效率为94．28％，对照组治疗总有效率为77．14％，观察组疗效显著优于对照组（P〈0．05）。治疗前两组患者的全血比粘度、红细胞压积、血浆比粘度和血红蛋白都没有统计学差异（P〉0．05），治疗后观察组患者的上述指标都显著优于对照组（P〈0．05）。结论低分子肝素联合酚妥拉明治疗肺心病顽固性心衰疗效确切、安全，值得临床推广使用。Objective To observe the clinical effects of low molecular heparin combined with phentolamine in treatment of cor pulmonale with refractory heart failure. Methods We analyzed the clinical effects of the low molecular heparin combined with phentolamine to the routine complex treatment （observation group, n=35） or the routine complex treatment （control group, n=35） for treatment of 70 patients with cor pulmonale with refractory heart failure in our hospital from May 2010 to November 2012. Results After treatment, whole blood contrast was （7.09 ± 1.25）, hematocrit was （0.541 ±0.114）, blood plasma viscosity was （1.62± 0.07）, hemoglobin was （121.31 ± 25.22） g/L, and clinical effect rate was 94.28% in the observation group. Whole blood contrast was （8.87 ± 1.13）, hematocrit was （0.592 ± 0.125）, blood plasma viscosity was （1.83 ± 0.11）, hemoglobin was （136.36 ± 36.63） g/L, and clinical effect rate was 77.14% in the control group. The clinical effects and blood rheology in the observation group were significantly better than those in the control group. Conclusion The low molecular heparin combined with phentolamine in treatment of cor pulmonale with refractory heart failure has definite efficacy and safety. It is worthy to be popularized in clinic.
