摘  要:目的探讨新生儿脐带血胰岛素样生长因子-Ⅰ(IGF-Ⅰ)与妊娠糖代谢异常母体糖化血红蛋白(HbAlc)水平相关性。方法本院分娩的足月单活胎孕母及其分娩的新生儿60例,分为GDM组、GIGT组、正常对照组,各20例。孕妇分娩前2周静脉采全血,Elisa法检测血清中HbAlc水平;新生儿娩出后,断脐采血,双位点免疫酶分析法检测血清中IGF-Ⅰ水平。对各组HbAlc水平、新生儿脐带血IGF-Ⅰ水平、新生儿出生体质量进行相关分析。结果 GDM组HbAlc水平明显高于正常对照组(P<0.05);GIGT组IGF-Ⅰ水平、新生儿出生体质量明显高于正常对照组(P<0.05)。正常对照组IGF-Ⅰ与出生体质量之间、HbAlc与IGF-Ⅰ之间呈明显正相关(P<0.05);GIGT组与GDM组IGF-Ⅰ与出生体质量之间呈明显正相关(P<0.05)。结论孕母妊娠末期血浆IGF-Ⅰ的检测在某种程度上能够反应胎儿内分泌因子IGF-I水平,IGF-I水平反映了胎儿宫内生长发育情况。Objective To investigate the correlation between insulin-like growth factor-Ⅰ(IGF-Ⅰ) in neonatal umbilical cord blood and glycosylated hemoglobin(HbAlc) in the matrix with impaired gestational glycometabolism.Methods The puerperal full-term single live-birth women and their puerperal neonates(n=60) were divided into GDM group,GIGT group and normal control group,20 cases in each group.The whole blood of pregnant women was drawn 2 weeks before delivery,and Elisa method was performed to detect the level of serum HbAlc.After the delivery,the umbilical cord was cut off to collect blood,and immunoenzymometric assay at two sites was performed to detect the level of serum IGF-Ⅰ.The relevant analysis was conducted on the levels of HbAlc and IGF-Ⅰin neonatal umbilical cord blood as well as neonatal birth weight in each group.Results The level of HbAlc in GDM group was obviously higher than that in normal control group(P〈0.05).The level of IGF-Ⅰ and neonatal birth weight in GIGT group were markedly higher than those in normal control group(P〈0.05).The relationships between IGF-Ⅰ and birth weight as well as HbAlc and IGF-Ⅰ in normal control group,IGF-Ⅰ and birth weight in GIGT and GDM groups were remarkably positive(P〈0.05).Conclusion Detection of plasma IGF-Ⅰ in late-pregnancy women can reflect the level of endocrine factor IGF-I in fetuses to a certain extent which reflects the intrauterine growth and development of fetuses.
