耳石复位法在分型诊治良性阵发性位置性眩晕中的应用

摘　　要：目的探讨耳石复位法(CRP)在分型诊治良性阵发性位置性眩晕(BPPV)中的应用价值。方法对73例BPPV患者进行分型、定侧,采取相应的CRP治疗。1周后对其复诊,评价疗效并再次行CRP治疗。3个月后随访评估疗效。结果 CRP治疗1周后复诊结果显示,73例患者中,前、后半规管性BPPV57例,采用改良Epley法治疗的有效率为87.7%;水平半规管性BPPV13例,采用Barbecue翻滚法治疗的有效率为76.9%;混合性半规管性BPPV3例,采用改良Epley复位手法+Barbecue翻滚法治疗的有效率为66.7%。3个月后随访评估疗效显示,57例前、后半规管性BPPV患者的有效率为93.0%,13例水平半规管性BPPV患者的有效率为84.6%,3例混合性半规管性BPPV患者的有效率为66.7%。结论针对不同类型的BPPV患者,采用相应的CRP治疗可以取得较好的疗效,再次CRP治疗可使疗效更为巩固和显著。Objective To investigate the application of canalith reposition procedure（CRP）in diagnosing and treating various benign paroxysmal positional vertigo （BPPV）.Methods After the subtype and the effect side were determined,the 73 patients with BPPV were treated with appropriate CRP.A week later return visit and curative effects evaluation were made,and treatment with CRP was given again.Three months after,the curative effects were estimated.Results Return visit one week after treatment with CRP showed that 57 out of the 73 patients with BPPV anterior and posterior canal involvement who were treated with improved Epley recovered,the effectiveness rate being 87.7 ％.The effectiveness rate of 13 patients with horizonial canal involvement treated with barbecue maneuver was 76.9 ％,and the effectiveness rate of 3 patients with mixed type treated with improved Epley＇ s maneuver combined barbecue maneuver was 66.7 ％.Following up and evaluation of the curative effect made after three months showed that the effectiveness rate of 57 patients with anterior and posterior canal involvement was 93.0 ％.The effectiveness rate of 13 patients with horizonial canal involvement 84.6 ％,and the effectiveness rate of three patients with mixed type 66.7 ％.Conclusion Patients with different BPPV types are treated with corresponding CRP,which can achieve good curative effects,and treatment with CRP again can make the curative effect much better.
