贮存式自体输血在前列腺手术中的血液流变学与免疫学观察

摘　　要：目的:探讨前列腺患者行贮存式自体输血适宜性,并观察血液采集前后对血液流变学与免疫学的影响。方法:选择36例需手术治疗并有可能术中输血的前列腺疾病患者,严格进行自体输血前适应证与禁忌证评估,观察血液流变学和免疫学检测在自体血采集前后变化。结果:本组患者血液流变学检测项目,切变率1.0、血沉、血细胞比容在自体血采集前后均差异无统计学意义,均P>0.05;切变率5.0、30、200和血浆黏度均差异有统计学意义,P<0.05或P<0.01。而免疫学检测项目,在自体血采集前后均差异无统计学意义,均P>0.05。结论:贮存式自体输血不仅适宜于前列腺患者的输血治疗,而且可显著改善这一人群的血液流变学异常状况,而对免疫学功能无明显影响。Objective：To discuss the suitability of preoperative autologous blood transfusion in patients with benign prostatic hyperplasia,and observe the influence of blood collection to the hemorheology and immune system.Method：The change of hemorheology and immunological detection before and after the autologous blood collection was observed by 36 cases of patients who suffered with benign prostate hyperplasia and need to adopt surgery treatment with the possibility of blood transfusion in the operation.Result：After the blood collection,the shear rate 5.0,shear rate 30,shear rate 200 and the plasma viscosity were significantly lower than these values were detected before the blood collection（P〈0.05,even P0.01）,when there was no statistically significant differences of shear rate 1.0,erythrocyte sedimentation rate（ESR）and hematocrit value（HCT）（P〈0.05）.And there was no statistically significant differences to all the values of the immunological test before and after the autologous blood collection（P〈0.05）.Conclusion：Preoperative autologous blood transfusion is not only suitable for the blood transfusion treatment in patiens with benign prostate hyperplasia,but also can significantly improve the abnormalities of hemorheology of this group,and there is no significant effect on the immunological function.
