摘  要:目的探讨早产小于胎龄(SGA)儿生后早期血清生长激素释放肽(Ghrelin)变化及其对生长发育的影响。方法选取生后24 h内入院的早产儿41例,分为早产SGA组19例和早产适于胎龄(AGA)组22例,采用酶联免疫吸附(ELISA)法测定两组生后24 h内和第10天血清Ghrelin水平,同时测定胰岛素样生长因子-1(IGF-1)、胰岛素(INS)水平,测量体格发育指标。结果①早产SGA组出生后24 h内血清Ghrelin水平明显高于早产AGA组(P<0.01),IGF-1、INS水平明显低于早产AGA组(P<0.05);②早产SGA组生后24 h内血清Ghrelin水平与出生体重、身长、BMI、IGF-1、INS呈负相关(P<0.05),INS与IGF-1水平呈正相关(P<0.01);③生后第10天,早产SGA组体重增长百分比、血清Ghrelin水平高于早产AGA组(P<0.05);两组IGF-1、INS水平比较,差异无统计学意义。结论 Ghrelin参与早产儿的宫内发育过程,可作为反映早产儿宫内发育和营养的重要指标,高水平Ghrelin可能有助于早产SGA儿出生后早期体格生长。Objective To explore the change of serum Ghrelin level and the effect on the growth and development of small for gestational age(SGA) preterm infants.Methods 41 preterm infants admitted to hospital 24 hours after birth were divided into two groups——SGA preterm infants(n=19)and appropriate for gestational age(AGA) preterm infants(n=22).The serum Ghrelin levels of them were tested by ELISA in 24 hours and on the tenth day after birth,and insulin like growth factor-1(IGF-1),insulin(INS) and anthropometric characteristic were measured at the same time.Results ① In the 24 hours after birth,the level of serum Ghrelin in SGA preterm infants was significantly higher than that in AGA preterm infants(P0.01),while the levels of serum IGF-1,INS were obviously lower than those in AGA preterm infants(P0.05).② The level of serum Ghrelin in SGA preterm infants was negatively correlated with birth weight,birth length,body mass index(BMI),IGF-1 and INS(P0.05);the level of serum INS was positively correlated with IGF-1(P0.01);③ On the tenth day after birth,both the weight growth percentage and the serum Ghrelin level in SGA preterm infants were higher than those of AGA preterm infants(P0.05),and the consentrations of serum IGF-1 and insulin were no significant difference in two groups.Conclusion Ghrelin is involved in intrauterine growth and development of preterm infants,which reflects nutritional status of fetal,and the high level of Ghrelin may be useful for physical growth in SGA preterm infants shortly after birth.
