胸膜外微创结扎动脉导管的临床应用

摘　　要：目的探讨小切口胸膜外动脉导管未闭结扎术的效果和价值。方法将48例单纯动脉导管未闭患者随机分为微创组及对照组各24例。微创组予经左听诊三角区小切口胸膜外动脉导管未闭结扎术,对照组按传统方法治疗。观察2组呼吸机辅助时间、输血量、重症监护室(ICU)滞留时间、术后住院时间、术后引流量及住院费用情况。结果微创组呼吸机辅助时间、ICU滞留时间及术后住院时间均短于对照组,输血量、术后引流量及住院费用均少于对照组,差异均有统计学意义(P<0.05或P<0.01)。结论小切口胸膜外动脉导管未闭结扎术具有创伤小、并发症少及恢复快的优点,是一种安全、经济、有效的手术方法,值得临床推广应用。Objective To investigate the curative effect and value of the minimally invasive ligation in patent ductus arteriosus. Methods 48 cases patients with simple ductus arteriosus were randomly divided into minimally invasive group and control group, each of 24 cases. Minimally invasive group were treated with left auscultation triangle small incision extrapleural ligation of patent ductus arteriosus, control group were given traditional treatment. Observed the ventilation time, blood transfu- sion,intensive care unit （ICU） residence time, postoperative hospital stay, postoperative drainage situation and hospitalization costs of two groups. Results The ventilation time, ICU residence time and postoperative hospital stay of minimally invasive group were shorter than those of control group;blood transfusion, postoperative drainage situation and hospitalization costs of minimally invasive group were less than those of control group, the differences were statistically significant （P 〈 0. 05 or P 〈 0. 01 ）. Conclusion Small incision extrapleural ligation of patent ductus arteriosus with less trauma, fewer complications and quick recovery,it＇s a safe,economical and effective surgical method,and worthy of clinical application.
