摘 要:目的评估第6秒用力呼气容积(forced expiratory volume in six seconds,FEV_6)替代用力肺活量(forced vital capacity,FVC)诊断中重度慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)的效果。方法对2010年8月—2012年8月河北省胸科医院287例舒张前、舒张后呼气时间达6s以上的中重度COPD患者的肺功能检查结果进行回顾性分析,对舒张后的FEV_6与FVC、FEV_1/FEV_6与FEV_1/FVC进行相关性分析,以FEV_1/FEV_6<70%为标准,判定诊断效果。结果 FEV_6与FVC、FEV_1/FEV_6与FEV_1/FVC高度相关(r=0.989,r_s=0.979,P<0.05)。以FEV_1/FEV_6<70%为诊断标准,287例患者中273例诊断为COPD,诊断率为95.12%。结论 FEV_1/FEV_6与FEV_1/FVC高度相关,FEV_1/FEV_6诊断中重度COPD与FEV_1/FVC具有高度一致性,FEV_6可以有效替代FVC用于诊断中重度COPD。Objective To determine the efficacy of forced expiratory volume in six seconds (FEV6 ) as an alternative for forced vital capacity (FVC) in the diagnosis of moderate-severe chronic obstructive pulmonary disease (COPD). Methods The spirometric examinations of 287 moderate-severe COPD patients were analyzed, FEV6 was measured on volume-time curves. The correlation between FEV6 and FVC,FEV1/FVC and FEV1/FEV6 was evaluated by Pearson test and Spearman test. FEV1/FEV6 〈 70% was considered as the standard for obstruction to evaluate the diagnosis effect. Results FEV~ and FVC,FEV1/FEV6 and FEVJFVC ratios were highly correlated ( r = 0. 989, rs = 0. 979, P 〈 0.05 ). Considering FEVJFEV6 〈 70% as the standard for obstruction, 95. 12% patients (273/287) were diagnosed COPD. Conclusion There are strong correlation between FEV6 and FVC, FEV1/FVC and FEVI/FEV6. FEV6 can be a valid alternative for FVC in the diagnosis of moderate-severe chronic obstructive pulmonary disease.
