摘  要:目的探讨肠道菌群变化、CD3^+CD8^-IL-17^+细胞在喘息样支气管炎患儿发病机制中的作用关系。方法采用荧光定量聚合酶链式反应(flurogenic quantitative polymerase chain reaction,FQ-PCR)、流式细胞技术和酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)分别检测47例喘息样支气管炎患儿、40例急性支气管炎患儿、43例同期健康查体儿童粪便中双歧杆菌和大肠杆菌数量、外周血单个核细胞CD3^+CD8^-IL-17^+浓度和血浆白细胞介素(interleukin-17,IL-17)水平。结果①喘息样支气管炎患儿和急性支气管炎患儿粪便中双歧杆菌数量、双歧杆菌/大肠杆菌(bifidobacteria/E.coli,B/E)值低于健康对照组(P<0.05);喘息样支气管炎患儿双歧杆菌数量、B/E值低于急性支气管炎患儿(P<0.05)。②喘息样支气管炎、急性支气管炎患儿外周血CD3^+CD8^-IL-17^+细胞百分数和血浆IL-17水平均高于健康对照组(P<0.05);喘息样支气管炎患儿外周血CD3^+CD8^-IL-17^+细胞百分数高于急性支气管炎患儿(P<0.05),血浆IL-17水平低于急性支气管炎患儿(P<0.05)。③喘息样支气管炎和急性支气管炎患儿外周血CD3^+CD8^-IL-17^+细胞百分数与血浆IL-17水平呈正相关。喘息样支气管炎患儿肠道B/E值与外周血CD3^+CD8^-IL-17^+细胞百分数呈负相关。结论喘息样支气管炎患儿发病初期即发生肠道菌群紊乱,肠道微生物定植抗力降低,CD3^+CD8^-IL-17^+细胞介导的炎症反应参与其病理机制。Objective To investigate the variations of intestinal flora and the relationship between intestinal flora and CD3+CD8-IL-17+ cells in children with asthmatoid bronchitis. Methods Forty-seven children with asthmatoid bronchitis, forty chilidren with acute tracheitis and forty-three healthy children were selected in the corresponding time period. The bifidobacteria and E. coli DNA in fecal samples were detected by fluorogenic quantitative polymerase chain reaction. The percentage of CD3+CD8-IL-17+ cells in peripheral blood mononuclear cells were detected by flow cytometrie analysis. Interleukin-17 (IL-17) in plasma was detected by enzyme linked immunosorbent assay. Results (1)The quantity of fecal bifidobacteria and bifidobacteria/E, coli( B/E ) values in children with asthmatoid bronchitis and acute tracheitis were lower than those in healthy children ( P 〈 0.05 ) ; The quantity of fecal bifidobacteria and B/E values in children with asthmatoid bronchitis were lower than those in children with acute tracheitis (P 〈 0.05 ). (2)The percentage of CD3+CD8-IL-17+ cells and the IL-17 level in children with asthmatoid bronchitis and acute tracheitis were higher than those in healthy children (P 〈 0.05 ). The percentage of CD3+CD8-IL-17+ cells were higher in children with asthmatoid bronchitis, but the IL-17 level was lower than that in children with acute tracheitis ( P 〈 0.05 ). (3)There was a positive linear corelation between the percentage of CD3+CD8-IL-17+ ceils and the IL-17 level in children with asthmatic bronchitis and acute tracheitis. There was a negative linear corelation between B/E ratio and the percentage of CD3+CD8-IL-17+ cells in children with asthmatic bronchitis. Conclusion The construction of intestinal flora in children with asthmatoid bronchitis were disordered and intestinal microbial colonization resistance decreased; CD3+CD8-IL-17+ cells in children with asthmatoid bronchitis are involved in the early period of disease and pl
