中医分期论治对咳嗽变异性哮喘患儿血清IFN-γ及IL-4的影响

摘　　要：目的:观察中医分期论治对咳嗽变异性哮喘(CVA)患儿的疗效及血清IFN-γ及IL-4的影响,并探讨其可能的作用机制。方法:将符合本研究标准的CVA患儿60例,随机分为治疗组和对照组各30例。治疗组发作期给予温肺化痰饮治疗,缓解期给予补肺健脾饮治疗;对照组服用孟鲁司特钠治疗,发作期加服氨茶碱治疗。观察两组临床疗效与治疗前后IFN-γ及IL-4的变化。结果:两组综合疗效总有效率均为93.4%,差异无统计学意义(P>0.05);但证候疗效总有效率治疗组为96.7%,对照组为90.0%,治疗组优于对照组(P<0.01);治疗后两组CVA患儿血清IFN-γ水平较治疗前均明显升高(P<0.01),IL-4水平较治疗前明显降低(P<0.01);且治疗后血清IFN-γ及IL-4水平治疗组优于对照组,两组比较,差异具有统计学意义(P<0.01)。结论:中医分期治疗对CVA患儿具有更好的疗效,其作用机制可能与显著升高IFN-γ和降低IL-4水平有关。Objective：To observe the therapeutic efficacy of traditional Chinese medicine （TCM） treatment by stage in children with cough variant asthma（CVA） and the effects of TCM treatment by stage on serum interferon gamma （IFN -γ） and interleukin 4 （IL -4） levels and to investigate its possible action mechanism. Methods：Sixty children with CVA were randomly divided into treatment group （n = 30） and control group （n = 30 ）. The treatment group received lung- warming and phlegm- eliminating decoction in acute stage and lung- nourishing and spleen -strengthening decoction in remission stage, while the control group received montelukast sodium plus amin- ophylline （given in acute stage）. The treatment outcomes and changes in serum IFN -γ and IL- 4 levels were ob- served. Results ： Both groups had an overall response rate of 93.4% , without significant difference between them （P 〉 0. 05 ）. The treatment group had a significantly higher overall syndrome improvement rate than the control group （96. 7% vs 90. 0% ,P 〈 0. 01 ）. After treatment, both groups showed a significantly increased serum IFN - γevel（P 〈 0.01 ） and a significantly decreased serum IL- 4 level（P 〈 0.01 ）, and the treatment group was signifi- cantly superior to the control group in terms of serum IFN - γ and IL -g levels （P 〈 0. 01 ）. Conclusion ：TCM treat- ment by stage has good therapeutic efficacy in children with CVA. Its action mechanism may be related to significant increase in serum IFN-γlevel and significant decrease in serum IL- 4 level.
