彩色多普勒超声在血管性阳萎诊断中的价值

摘　　要：目的 :探讨彩色多普勒超声诊断血管性阳萎的应用价值。方法 :应用彩色多普勒超声检查 ,对临床疑阳萎 39例患者的阴茎海绵体动脉血流进行检测 ,观测阴茎海绵体内注射罂粟碱加立其丁之前、后的动脉管径值及注药后 15 min内的收缩期峰值流速 ( Vmax)、舒张末期最小流速 ( Vmin)、阻力指数 ( RI)、搏动指数 ( PI)。结果 :对罂粟碱加立其丁反应良好 7例 ,反应低下 32例 ,反应良好者 Vmax为 ( 41.33± 9.76 ) cm/ s,Vm in为 ( 3.5 2± 1.2 2 ) cm/ s,RI为 0 .94± 0 .0 4,PI为 4.87± 0 .6 0 ;反应低下者中 18例为静脉机能不全 ,10例为动脉供血不足 ,4例为动静脉混合性阳萎 ,其 Vmax分别为 ( 36 .14± 8.2 ) cm / s、 ( 2 2 .2 3± 4.6 ) cm/ s、 ( 2 3.5 7± 5 .0 ) cm/ s,Vmin分别为 ( 7.2 0± 2 .10 ) cm/ s、 ( 2 .5 1± 1.31) cm/ s、 ( 6 .6 8± 1.40 ) cm/ s,RI分别为 0 .75± 0 .0 4、 0 .89± 0 .0 6、 0 .6 6± 0 .0 6 ,PI分别为 2 .0 8± 0 .5 6、 4.5 2± 0 .5 1、 2 .15± 0 .5 5。反应良好与反应低下的静脉机能不全者注药前后阴茎海绵体动脉管径值的增加率大于 2倍 ,而动脉供血不足与动静脉混合性者注药前后阴茎海绵体动脉管径值的增加率小于 6 0 %。结论 :彩色多普勒超声配合罂粟碱加立其丁试?Objective:To observe the value of color Doppler flow imaging in diagnosis of patients with vascular impotenceMethods:The penis cavernosal artery flow of 39 patients with doubtful impotence were detected by color Doppler ultrasonicThe artery canal diameters of penis cavernosal were determined before and after injection with papaverine and regitineThe peak flow velocity of systole (Vmax), minimal velocity of diastolic last stage (Vmin),resistant index(RI) and pulsation index(PI) were measured in 15 minutes injection with drugResults:7 patients had good response with Vmax (4133±976)cm/s,Vmin(352±122)cm/s,RI 094±004 and PI 487±060;Among the poorly responsed patients,18 cases were vein function incompetence,10 cases were artery blood supply insufficiency and 4 cases were artery vein mixing property impotence,their Vmax were (3614±82)cm/s、(2223±46)cm/s and (2357± 50)cm/s,Vmin were (720±210)cm/s、(251±131)cm/s and (668±140)cm/s,RI were 075±004、089±006 and 066±006,PI were 208±056、452±051 and 215±055,respectivelyThe penis cavernosal artery canal diameters of patients with vein function incompetence were larger two folds than that before injection with drug,but the patients with artery blood supply insufficiency and artery-vein leak were only larger 60% after drug injectionConclusion:The color Doppler flow imaging combination with papaverine and regitine are important examinations for diagnosis of vascular impotence
