开通玄府、通络解毒法治疗斑块状银屑病120例疗效观察

摘　　要：目的评价开通玄府、通络解毒法治疗斑块状银屑病的临床疗效和安全性。方法将150例患者按照4∶1比例随机分为治疗组120例和对照组30例,治疗组口服开通玄府通络解毒法中药,对照组口服活血解毒法协定处方,均每日1剂。连续用药8周后,评价临床疗效,并观察治疗前及治疗2周、4周、8周皮损面积和严重程度指数(PASI)评分、瘙痒程度评分、不同部位(头部、上肢、躯干、下肢)皮损评分、靶皮损不同症状(红斑、鳞屑、浸润)评分变化。结果治疗组和对照组总有效率分别为81.89%和39.29%,差异具有统计学意义(P<0.01)。两组治疗后2周、4周、8周皮损PASI评分、瘙痒程度评分及靶皮损红斑、鳞屑、浸润评分均低于本组治疗前,并且两组间治疗后同时间点比较差异均有统计学意义(P<0.05或P<0.01)。两组治疗后2周、4周、8周头部、上肢部位皮损评分组间比较差异有统计学意义(P<0.05),躯干、下肢部位皮损评分治疗后2周、4周、8周均低于本组治疗前,并且治疗后同时间点组间差异均有统计学意义(P<0.05)。两组均无明显不良反应。结论开通玄府、通络解毒法治疗斑块状银屑病临床疗效确切,可以改善皮损及瘙痒程度,安全性良好。Objective To evaluate the efficacy and safety of sweat pore-opening, network vessels-freeing and toxin-resolving therapy for plaque psoriasis. Methods Totally 150 cases of plaque psoriasis were randomized into the treatment group of 120 cases and control group of 30 cases. The treatment group was given sweat pore-opening formula and the control group was given blood-quickening toxin-resolving formula one dose daily for 8 weeks. The efficacy was evaluated. The scores of Psoriasis Area and Severity Index （PASI）, pruritus, skin lesions in different parts of the body and target lesions were observed before and after 2-week, 4-week and 8-week treatment. Results The total effective rate of the treatment group and control group was 81.89 % and 39.29 % respectively, with a significant difference between groups （P〈0.01）. There were significant differences between groups and within groups in the scores of PASI, pruritus and target lesions at different time points （P〈0.05 or P〈0.01）. There were significant differences between groups in the scores of PASI in head and upper limbs at different time points （P〈0.01）. There were significant differences Between groups and within groups in the scores of PASI in trunk and lower limbs at different time points （P〈0.05）. There was no significant adverse reaction. Conclusion The sweat pore-opening, network vessels-freeing and toxin-resolving therapy is effective for plaque psoriasis. It can obviously relieve skin lesions and pruritus with a better safety.
