摘  要:目的探讨重型颅脑外伤后血管痉挛与脑脊液NO及内皮素水平的关系。方法对重型颅脑外伤68例患者在入院后24h、第3天、第5天及第7天进行双侧大脑中动脉、颈内动脉颅外段脑血流速度检测及脑脊液中NO及内皮素水平进行测定。结果患者中有54.4%(37例)发生脑血管痉挛,重度16例,中度11例,轻度10例。检测发现,重型脑外伤后患者脑脊液内皮素浓度均升高,而NO水平却明显下降。脑血管痉挛组内皮素与NO水平变化与经颅多普勒检查相一致。重度和中度痉挛组与轻度痉挛组NO和内皮素水平比较差异有统计学意义(P〈0.05)。结论重度颅脑外伤容易发生脑血管痉挛,可能与脑脊液中内皮素水平升高和NO水平降低有关。Objective To investigate the lin in patients with severe traumatic brain injury. correlation of cerebral vasospasm with NO and endothe- Methods The cerebral blood flow velocity of bilateral middle cerebral artery, extracranial internal carotid artery segment were detected, NO and endothelin lev- els in eerebrospinal fluid of 68 patients with severe brain traumatic injury were measured at 24 hours, 3 days, 5 days and 7 days after admission. Results 54. 4% (37 cases) of patients with cerebral vaso- spasm, severe in 16 cases, moderate in 11 cases, mild in 10 cases. Patients with severe brain injury showed decreased concentration of NO and increased level of ET. Conclusions Patients with severe traumatic brain injury is prone to have cerebral vasospasm, it may be associated with elevated endothelin and decreased NO levels in the cerebrospinal fluid.
