住院精神病患者使用阿立哌唑药物调查

摘　　要：目的了解某院住院精神病患者使用阿立哌唑药物的情况,为医院管理提供科学依据。方法采用现况调查法对该院的169例住院精神患者的一般情况、诊断和治疗情况进行调查,对其中使用阿立哌唑的临床病例进行深入分析。结果 169例病人均使用精神药物,其中使用阿立哌唑34例,占精神药物使用频度的第三位(20.12%);第一位利培酮73例(43.20%),第二位氯氮平61例(36.10%);特殊治疗应用很少,仅6例行现代改良电休克治疗(MECT)。使用阿立哌唑的女性多于男性,年龄在15～54岁,平均(34.5±8.74)岁。住院天数(39.03±27.39)d。初发和复发患者均使用,主要用于治疗精神分裂症,占82.35%,其次是心境障碍占14.71%。单用阿立哌唑2例(5.89%),联合一种精神药物14例(41.18%),联合二种精神药物11例(32.35%),联合3种或以上精神药物7例(20.59%)。每天服1次7例(20.59%),每天服2次27例(79.41%)。联用苯海索9例(26.47%)。结论阿立哌唑使用基本能按说明书和循证医学结论使用,但仍需进一步完善阿立哌唑的合理使用。OBJECTIVE To investigate the application of aripiprazole in hospitalized psychosis patients and provide scientific bases for hospital management. METHODS This study adopted investigation method. The general situation, diagnoses and therapy of 169 hospitalized psychosis patients were investigated. Patients used aripiprazole were analyzed in depth. RESULTS All 169 patients used antipsyehotic drugs. There were 34 patients used aripiprazole and the utility rate was 20.12%. The utility rates of risperidone and clozapine were 43.20% and 36.10%. Only 6 patients used modified electroconvulsive therapy （MECT）. The use of aripiprazole in patients showed differences between men and women. The first episode patients and recur- rent patients both used aripiprazole. Aripiprazole was mainly used in the treatment of schizophrenia （82.35%） and mood dis- orders （14.71%）. There were 2 patients （5.89%） singly using aripiprazole. 14 patients （41.18%） used another kind of an- tipsychotic drugs; 11 patients （32.35%） combined use of two kinds of antipsychotic drugs; 7 patients （20.59%） combined use of three kinds of antipsychotie drugs. The combined use of trihexyphenidyl was found in 9 patients （26.47%）. There were 7 patients （20.59%） took aripiprazole once a day, other 27 patients （79.41%） twice a day. CONCLUSION The use of aripiprazole is basically used according to drug instructions and conclusion of evidence based medicine. But the rational use of aripiprazole needs to be consummated yet.
