摘 要:目的探讨过氧化物酶增殖体激活受体-δ(PPAR-δ)基因多态性与淮海地区汉族人群2型糖尿病(T2DM)及代谢指标的相关性,以期进一步了解T2DM发病的遗传学机制。方法采用聚合酶链反应-限制性片断长度多态性(PCR-RFLP)方法对200例T2DM患者(病例组)和200例健康人群(对照组)进行基因多态性检测。同时测定两组人群的身高、体重、腰围、臀围、收缩压(SBP)、舒张压(DBP),空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)等临床指标,计算体质指数(BMI)和腰臀比。结果 PPAR-δ-87T/C位点基因型和等位基因频率在病例组和对照组间差异均无统计学意义(P>0.05)。此位点不同基因型T2DM患者的体重和BMI差异有统计学意义(P<0.05);两两比较结果显示,CC型的体重([76.75±11.10)kg]显著高于TT型([68.43±13.10)kg],CC型的BMI([27.99±3.62)kg/m2]显著高于TT和TC型[分别为(25.10±3.71)、(25.69±3.40)kg/m2],差异有统计学意义(P<0.05,P<0.01)。其他指标如年龄、身高、腰围、臀围、腰臀比、SBP、DBP、FPG、TC、TG、HDL-C、LDL-C在3组基因型患者中差异均无统计学意义(P>0.05)。结论 PPAR-δ-87T/C多态性可能与中国淮海地区汉族人群2型糖尿病的发病无关,其多态性可能与2型糖尿病患者的肥胖发生有一定相关性。Objective To investigate the correlation between PPAR-δ gene polymorphisms,type 2 diabetes mellitus and metabolic indicators in Chinese Han population of Huaihai area.Methods The subjects were divided into case group(200 patients with type 2 diabetes mellitus)and control group(200 healthy people).The PPAR-δ gene polymorphisms in all subjects were detected with polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)method.Heigh,weight,waist circumference(WC),hip circumference(HC),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FPG),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C)were measured,and the body mass index(BMI)and waist to hip ratio(WHR)were calculated in all subjects.Results There were no significant differences of the genotype frequency and allete frequency at PPAR-δ-87T/C site between two groups(P0.05).There were significant differences of weight and BMI between different genotypes in patients with type 2 diabetes mellitus(P0.05).The weight of cases with CC genotype was significantly higher than that of case with TT genotype,the BMI of cases with CC genotype was significantly higher than that of case with TT and TC genotypes(P0.05 or P0.01).There were no significant differences of other indexes among cases with 3 different genotypes(P0.05).Conclusion There may be not the obvious correlation between the polymorphisms of PPAR-δ-87T/C and type 2 diabetes mellitus for Han redidents in Huaihai area,but PPAR-δ-87T/C gene polymorphisms may be associated with the obesity of patients with type 2 diabetes mellitus.
