多水平模型及其在社区人群心理障碍影响因素研究中的应用

摘　　要：目的了解厦门市社区人群心理障碍水平及其相关因素。方法利用厦门市18岁及以上人群精神障碍流行病学调查部分资料,采用多水平回归模型拟合10762名调查对象的相关因素与心理障碍的相互关系;心理障碍评定采用一般健康问卷(GHQ-12)。结果厦门市社区人群心理障碍的平均得分为(0.64±1.55)分,单因素分析显示与心理障碍问题相关的因素为性别、户籍、婚姻状况、职业、居住方式、医疗来源、年家庭人均收入;厦门市社区人群心理障碍在乡镇(街道)存在聚集性(χ2=4.260,P=0.039),组内相关系数ρ=0.046;与心理障碍相关因素有性别、年龄、居住方式、目前职业、医疗来源、年家庭人均收入、婚姻状况等,独居或其他、无业或失业、无医疗来源、家庭收入低、分居、离婚或丧偶为心理障碍的危险因素。结论厦门市社区人群心理障碍水平较低,且存在乡镇(街道)地域聚集性;独居或其他、分居、离婚或丧偶、无业或失业、无医疗来源和低收入人群是重点关注人群。Objective To explore the prevalence of mental disorders and its related factors in community setting in Xiamen city.Methods A multilevel model was analyzed using data of 10762 respondents nested in townships(urban districts),villages(urban neighborhoods) from the epidemiological survey of mental disorders aged 18 and older.Mental disorders were measured by GHQ-12.Results The mean score of mental disorders was(0.64 ± 1.55),and analysis of risk factors by nonparametric test showed that gender,household register,marital status,occupation,residency patterns,medical sources and household incoming.The mental disorders clustered within townships(urban districts) level,which represented 4.6%(χ2= 4.260,P = 0.039) of the total variance including three levels.The mental disorders were associated with gender,age,residency patterns,occupation,medical source,household incoming,marital status,and living alone,unemployed,no medical source,lower household incoming,separated,divorced and widowed were the risk factors of mental disorders.Conclusion The prevalence of mental disorders was relative lower,and clustered at the townships(urban districts) level.Persons in living alone,separated,divorced,widowed,unemployed,lower income were the important group of the mental disorders.
