卡前列甲酯栓用于绝经后妇女宫内节育器取出术的临床观察

摘　　要：目的将卡前列甲酯栓用于绝经后妇女宫内节育器(intrauterine contraceptive device,IUD)取出术。方法将2009年1月至2011年10月绝经时间5年以上、放置IUD的妇女按随机表法分成观察组与对照组各60例。观察组于术前2小时阴道后穹窿放置卡前列甲酯栓(0.5mg)1枚;对照组则不用任何药物。结果观察组42例取IUD顺利,17例扩张宫颈后取出,1例因IUD嵌顿子宫肌层较深,转外院行宫腔镜手术;对照组31例顺利取出IUD,18例扩张宫颈后取出,11例宫颈萎缩探针不能通过,或宫颈萎缩不明显但不能扩张。观察组取器成功率明显高于对照组,差异有统计学意义(P<0.05)。结论术前2小时使用卡前列甲酯栓后取器,取出成功率高,效果良好,可在临床上推广使用。Objective To evaluate Carboprost Methylate suppositories for removal of intrauterine contraceptive device（IUD） in post - menopause women. Methods A total of 120 women who had menopaused more than 5 years and prepared to remove IUD from January 2009 to October 2001 were randomly divided into two groups. About 60 cases were inserted Carboprost Methylate suppositories in posterior fornix of vagina 2 hours before the surgery in study group while the other 60 cases were given no intervention in control group. Results Among 60 cases in study group, 42 women got a successfully surgery, 17 women needed to use cervical dilator device, 1 case was failure because the exploring needle could not pass the cervix. And 31 women of the control group got a successfully sugery, 18 women needed to use cervical dilator device, 11 cases were failure because the exploring needle could not pass the cervix. The successful rate of removal IUD was significant differences between the two groups （P 〈 0.05）. Conclusion The application of Carboprost Methylate suppositories in posterior fornix of vagina 2 hours before surgery is suggested to be used for removal of intrauterine contraceptive device in post - menopause women. It has high success rate.
